糖尿病足溃疡患者近期预后情况及其影响因素分析

Short-term prognosis of patients with diabetic foot ulcer and its influencing factors

目的 探究糖尿病足溃疡(DFU)患者近期预后情况及其影响因素。方法 回顾性分析2017年1月至2020年1月本院收治的120例DFU患者的临床资料,根据预后结局分为预后良好组(n=35)和预后不良组(n=85)。采用单因素分析和多因素Logistic回归分析分析影响DFU患者近期预后情况的危险因素。结果 120例患者中,预后良好的患者35例,良好率29.17%,预后不良的患者85例,不良率70.83%;单因素分析结果显示,病程、周围神经病变情况、缺血情况、感染程度、动脉病变、空腹血糖、糖化血红蛋白(HbAlc)、溃疡面积、溃疡数目、低蛋白症是影响DFU患者近期预后的相关因素;多因素Logistic回归分析结果显示,病程、周围神经病变情况、缺血情况、感染程度、HbAlc、水肿、溃疡面积、低蛋白血症是影响DFU患者近期预后的危险因素。结论 病程、周围神经病变情况、缺血情况、感染程度、HbAlc、溃疡面积、HDL-C、低蛋白血症是影响DFU患者近期预后情况的危险因素,根据患者实际情况制定干预措施,以改善患者近期预后情况。

Objective To explore the short-term prognosis of patients with diabetic foot ulcer(DFU) and its influencing factors.Methods The clinical data of 120 DFU patients admitted to our hospital from January 2017 to January 2020 were retrospectively analyzed,they were divided into the good prognosis group(n=35) and the poor prognosis group(n=85) according to the prognosis.Univariate analysis and multivariate logistic regression analysis were performed to screen the risk factors influencing the short-term prognosis of patients with DFU.Results Among the 120 patients,there were 35 patients with good prognosis,with good rate of 29.17%,and 85 patients with poor prognosis,with poor rate of 70.83%;univariate analysis showed that the course of disease,peripheral neuropathy,ischemic state,degree of infection,arterial diseases,fasting blood glucose,glycosylated hemoglobin(HbAlc),ulcer area,number of ulcers,and hypoproteinemia were related factors influencing the short-term prognosis of patients with DFU;multivariate Logistic regression analysis showed that the course of disease,peripheral neuropathy,ischemic state,degree of infection,HbAlc,edema,ulcer area,and hypoproteinemia were risk factors influencing the short-term prognosis of patients with DFU.Conclusion Course of disease,peripheral neuropathy,ischemia,degree of infection,HbAlc,ulcer area,hypoalbuminemia were risk factors affecting the short-term prognosis of DFU patients,intervention measures are formulated according to the actual situation of patients to improve the short-term prognosis of patients.
