腹腔镜辅助小切口胃癌根治术治疗进展期胃癌的疗效及对患者术后恢复的影响

Efficacy of laparoscopic-assisted small incision radical gastrectomy for advanced gastric cancer and its effect on postoperative recovery.

目的 探究腹腔镜辅助小切口胃癌根治术治疗进展期胃癌的疗效及对患者术后恢复的影响。方法 选取2019年1月至2020年7月本院收治的80例进展期胃癌患者作为研究对象,按照手术方式的不同分为对照组和观察组,每组40例。对照组行传统开腹根治术,观察组行腹腔镜辅助小切口胃癌根治术,比较两组手术相关指标、术后疼痛及并发症发生率。结果 观察组手术时间、术后下床活动时间、术后肛门排气时间、术后首次进食时间及住院时间均短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05)。术后1、3、5 d,观察组视觉模拟评分法(VAS)评分均低于对照组,差异有统计学意义(P<0.05)。观察组并发症发生率为5.00%,明显低于对照组的20.00%,差异有统计学意义(P<0.05)。结论 进展期胃癌患者采取腹腔镜辅助小切口胃癌根治术治疗效果显著,可缩短患者的术后恢复时间,缓解患者疼痛,且并发症少,安全性高,值得临床推广应用。

Objective To explore the efficacy of laparoscopic-assisted small incision radical gastrectomy for advanced gastric cancer and its effect on postoperative recovery.Methods 80 patients with advanced gastric cancer who were treated in our hospital from January 2019 to July 2020were selected as the research subjects,and they were divided into control group and observation group according to different surgical methods,with40 cases in each group.The control group underwent traditional open radical resection,while the observation group underwent laparoscopic-assisted small incision radical gastrectomy,the operation-related indicators,postoperative pain and the incidence of complications were compared between the two groups.Results The operation time,postoperative ambulation time,postoperative anal exhaust time,postoperative first eating time and hospitalization time in the observation group were shorter than those in the control group,and the intraoperative blood loss was less than that in the control group,and the differences was statistically significant(P<0.05).At 1,3,and 5 d after operation,the VAS scores of the observation group were lower than that of the control group,and the differences was statistically significant(P<0.05).The incidence of complications in the observation group was5.00%,which was significantly lower than 20.00% in the control group,and the difference was statistically significant(P<0.05).Conclusion Laparoscopic assisted small incision radical gastrectomy for patients with advanced gastric cancer has significant therapeutic effect,which can shorten the postoperative recovery time of patients,relieve the pain of patients,with fewer complications and high safety,and is worthy of clinical application.
