血清miR-200c-3p、CA125与组织Id1联合检测对卵巢癌早期筛查和诊断的临床价值 被引量:6

摘要 目的探讨血清微小RNA-200c-3p(miR-200c-3p)、糖类抗原125(CA125)与组织分化抑制因子1(Id1)联合检测在卵巢癌早期筛查和诊断中的临床价值。方法将南京医科大学附属妇产医院2016年1月至2020年12月就诊的卵巢疾病患者128例分为卵巢良性肿瘤组40例和卵巢恶性肿瘤组78例,选择同期健康卵巢为对照组(40例)。分别采用实时荧光定量PCR和电化学发光法检测血清miR-200c-3p和CA125水平,免疫印迹法检测组织Id1水平。Pearson相关法分析卵巢癌患者血清miR-200c-3p、组织Id1水平与血清CA125水平的相关性。应用受试者工作特征(ROC)曲线分析miR-200c-3p、Id1及CA125水平诊断卵巢癌的价值。结果恶性肿瘤组的血清miR-200c-3p、CA125和组织Id1水平均高于良性肿瘤组和对照组(P<0.05);相关分析显示,卵巢癌患者的血清miR-200c-3p和组织Id1水平与血清CA125水平均呈正相关(r=0.734、0.707,P<0.05)。卵巢癌患者的血清miR-200c-3p、CA125和组织Id1水平与FIGO分期、分化程度、淋巴结转移和恶性腹水有关(P<0.05)。ROC曲线显示,血清miR-200c-3p、CA125和组织Id1水平三项联合诊断卵巢癌的曲线下面积(0.871,95%CI:0.812~0.922)最大,其敏感度、特异度和诊断效率分别为87.1%、88.5%和88.0%。结论血清miR-200c-3p、CA125与组织Id1联合检测能够显著提高卵巢癌早期筛查和诊断的效率,对于卵巢癌的临床早期筛查和诊断具有重要价值。 Objective To investigate the clinical value of serum microRNA-200c-3p(miR-200c-3p)and carbohydrite antigen 125(CA125)combined with tissue differentiation inhibitor 1(Id1)in early screening and diagnosis of ovarian cancer.Methods One hundred and twenty-eight patients with ovarian diseases selected from January 2016 to December 2020 in the Obstetrics and Gynecology Hospital Affiliated to Nanjing Medical University were divided into 40 benign ovarian tumors(benign tumor group)and 78 ovarian malignant tumors(malignant tumor group),and the healthy ovaries were selected as the control group(40 cases).Serum levels of miR-200c-3p and CA125 were detected by real-time fluorescent quantitative PCR and electrochemiluminescence,and Id1 level in tissues was detected by Western blot.Pearson correlation analysis was applied to investigate correlations of serum CA125 with serum miR-200c-3p and tissue Id1 levels in patients with ovarian cancer.Diagnostic value of serum miR-200c-3p and CA125 combined with tissue Id1 in ovarian cancer was analyzed by receiver operating characteristic(ROC)curve.Results Serum levels of miR-200c-3p and CA125 and tissue level of Id1 were significantly higher in the malignant tumor group than those in the benign tumor group and control group(P<0.05).Correlation analysis showed that the miR-200c-3p level in serum and Id1 in tissues of patients with ovarian cancer were positively correlated with CA125 level in serum(r=0.734 and 0.707,P<0.05).Levels of miR-200c-3p and CA125 in serum and Id1 in tissues of patients with ovarian cancer was related to FIGO stage,degree of differentiation,lymph node metastasis and malignant ascites(P<0.05).ROC curve showed that the area under the curve of miR-200c-3p,Id1 and CA125 for the combined diagnosis of ovarian cancer was the largest(0.871,95%CI:0.812-0.922)with a sensitivity of 87.1%,a specificity of 88.5%,and a diagnostic efficiency of 88.0%.Conclusion Combined detection of serum miR-200c-3p and CA125 with tissue Id1 can significantly improve the efficiency of early screening and diagnosis of ovarian cancer,and it is of great significance for early clinical screening and diagnosis of ovarian cancer.
