早期饮食和喂养行为干预对预防巨大儿肥胖的作用

摘要 目的:探讨早期饮食和喂养行为干预对预防巨大儿肥胖的作用。方法:本研究纳入出生的巨大儿90例,随机分为对照组和干预组各45例,对照组采用常规儿童保健干预,干预组在常规儿童保健干预的基础上于出生后1周和1个月、2个月、3个月及6个月对婴儿及其抚养人进行早期饮食和喂养行为指导和干预。收集并比较两组婴儿的身长、体重、头围、上臂围、腹围、胸围等体格检查指标,测定婴儿运动发育能力和搜集母乳喂养情况,同时在1月龄、3月龄和6月龄时对婴儿进行血液实验室检查。结果:干预组巨大儿的身长、体重、头围、上臂围、腹围、胸围均低于对照组且接近正常水平,运动发育能力明显优于对照组,母乳喂养成功率(64.4%)显著高于对照组的53.3%,血液实验室检查中各项指标优于对照组且更接近正常水平,两组比较差异均有统计学意义(P<0.05)。结论:早期饮食和喂养行为干预可以预防巨大儿发生肥胖,对降低巨大儿肥胖率有着积极作用。 Objective To investigate the effect of early dietary and feeding behavior intervention on prevention of macrosomia obesity.Method Ninety patients with macrosomia were enrolled in this study. They were randomly divided into control group and intervention group, 45 cases in each group. Control group using conventional treatment in child care, intervention group on the basis of routine treatment in child care in 1 week after birth and 1, 2, 3 and 6 months for baby and raise people early diet and feeding behavior guidance and intervention, collect and compare the two groups of the baby′s body length, body weight, head circumference, upper arm circumference, sebum thickness under 6, the chest, etc, which are the indices of physical examination, Motor development and breastfeeding were measured, and biochemical blood tests were performed at 1, 3 and 6 months of age.Results The body length, body weight, head circumference, upper arm circumference, abdominal circumference, chest circumference in the intervention group were lower than those in the control group and close to the normal level. The motor development ability of the intervention group was better than that of the control group. The success rate of breast-feeding in intervention group was 64.4%, significantly higher than 53.3% in control group. The indexes of blood biochemical examination in the intervention group were closer to the normal level, which was better than that in the control group. The differences were statistically significant(P<0.05).Conclusion Early intervention of diet and feeding behavior can prevent obesity in macrosomia and reduce obesity rate in macrosomia.
