摘要
目的探讨CXCR2蛋白在非特殊型浸润性乳腺癌(non-specific invasive breast carcinoma,IBC-NST)组织中的表达及临床病理意义。方法收集存档的乳腺腺病组织50例(乳腺腺病组),IBC-NST 87例(IBC-NST组),其中无淋巴结转移IBC-NST 44例(无淋巴结转移组)和有淋巴结转移IBC-NST 43例(淋巴结转移组)。应用免疫组化方法检测CXCR2蛋白的表达,同时对乳腺癌原发灶进行ER、PR、HER2、Ki-67免疫组化检测,对HER2(++)进行FISH检测,并根据2013年ST Gallen共识对乳腺癌原发灶进行分子分型(Luminal A型、Luminal B型、HER2过表达型、三阴性乳腺癌(triple negative cancer,TNBC),并分析CXCR2蛋白与不同分子分型IBC-NST发生的作用,及与淋巴转移的关系。结果IBC-NST组的CXCR2蛋白表达阳性率低于乳腺腺病组,差异有统计学意义(P<0.05),淋巴结转移组与无淋巴结转移组比较差异无统计学意义(P>0.05)。与其他分子分型比较,CXCR2在TNBC中表达最低,且差异有统计学意义(P<0.05),其他3种类型相比较无明显统计学意义(P>0.05)。结论CXCR2蛋白在乳腺腺病、无淋巴结转移和伴有淋巴结转移IBC-NST中均有表达,阳性程度逐渐递减,提示CXCR2蛋白的表达与IBC-NST的发生、发展密切相关;CXCR2蛋白在IBC-NST中表达降低,在TNBC中表达最低,提示CXCR2蛋白表达越低,可能侵袭性越高,与乳腺癌的淋巴转移可能有一定的关系。
Objective To investigate the expression of CXC chemokine receptor 2(CXCR2)in invasive breast carcinoma of no specific type(IBC-NST)and its significance.Methods The immunochemistry system was used to detect the expression of CXCR2 in 50 specimens from breast adenopathy tissues,44 specimens of non-lymph node metastasis in IBC-NST,and 43 specimens of lymph node metastasis in IBC-NST.ER,PR,HER2 and Ki-67 were assessed on the primary breast cancer.Those with HER2(++)were further analyzed with FISH for molecular typing,including Luminal A,Luminal B,HER2 overexpression,triple negative breast cancer(TNBC),according to the 2013 ST Gallen consensus.The role of CXCR2 protein in the occurrence of IBC-NST and its correlation with lymphatic metastasis was analyzed.Results Compared with the IBC-NST group,the positive rate of CXCR2 protein expression was significantly higher in the breast adenopathy group(P<0.05).There was no significant difference in CXCR2 protein expression between IBC-NST with lymph node metastasis and non-lymph node metastasis(P>0.05).Compared with other molecular types,the expression of CXCR2 in TNBC was significantly the lowest(P<0.05),while no significant difference was revealed among the other 3 types(P>0.05).Conclusion CXCR2 protein is expressed in breast adenopathy tissue,and IBC-NST without or with lymph node metastasis;and the degree of positivity gradually decreases,indicating that CXCR2 protein is closely correlated to the occurrence,development,and malignant transformation of IBC-NST.CXCR2 protein expression decreases in IBC-NST,and is the lowest in TNBC,suggesting that the lower the expression of CXCR2 protein,the higher the invasiveness.
