疏肝化瘀法治疗活动期甲状腺功能亢进性突眼临床观察

Observation on the Therapeutic Effect of Dispersing Stagnated Liver Qi for Removing Blood Stasis Method in the Treatment of Hyperthyroidism Exophthalmos in Active Stage

目的回顾性分析疏肝化瘀法治疗活动期甲状腺功能亢进性突眼的临床疗效。方法选取2015年5月—2019年5月就诊于辽宁中医药大学附属第二医院眼科患者68例(132眼),根据治疗方法随机分为2组,其中治疗组34例(67只眼)给予“疏肝化瘀法”(自拟疏肝化瘀汤口服、针刺、直流电离子导入)治疗,对照组34例(65只眼)给予直流电离子导入治疗,对照2组治疗前,治疗后4周、8周、12周的疗效、临床活动度评分(Clinical activity score,CAS)、眼球突出度变化情况进行分析评价。结果临床疗效:2组临床疗效比较,差异有统计学意义(P<0.01);2组CAS和突眼度:治疗后4周、8周、12周均明显降低(均P<0.05),且治疗组各指标改善程度均明显优于对照组(均P<0.05)。结论疏肝化瘀法能有效改善活动期甲状腺功能亢进性突眼患者CAS、眼球突出症状,且疗效显著。

Objective To retrospectively analyze the clinical effect of dispersing stagnated liver qi for removing blood stasis method in the treatment of hyperthyroidism exophthalmos in active stage.Methods From May 2015 to May 2019,68(132 eyes)ophthalmic patients were admitted to the Second Affiliated Hospital of Liaoning University of Traditional Chinese Medicine were randomly divided into two groups according to the treatment method.Among them,34 cases(67 eyes)in the treatment group were treated with dispersing stagnated liver qi for removing blood stasis method(self-made Shugan Huayu decoction,acupuncture,direct current iontophoresis),and 34 cases(65 eyes)in the control group were treated with direct current iontophoresis.The therapeutic effect,clinical activity score(CAS)and the changes of ocular protrusion at 4,8 and 12 weeks after treatment were analyzed and evaluated.Results The clinical effective rate in the treatment group,and the clinical effective rate in the control group.The difference in clinical efficacy between the two groups was statistically significant(P<0.01).The two groups of CAS and exophthalmos:4 weeks,8 weeks,12 weeks after treatment were significantly reduced(P<0.05),and the improvement of each index in the treatment group was significantly better than the control group(P<0.05).Conclusion The dispersing stagnated liver qi for removing blood stasis method can effectively improve CAS and exophthalmos in patients with active hyperthyroidism exophthalmos,and the curative effect is significant.
