血府逐瘀汤联合体外反搏治疗心血瘀阻型冠心病心绞痛

Effect of Xuefu Zhuyu Decoction combined with external counterpulsation in the treatment of angina pectoris of coronary heart disease with the pattern of stagnant blockade of heart blood

目的 观察血府逐瘀汤联合体外反搏治疗心血瘀阻型冠心病心绞痛的临床疗效。方法 选取心血瘀阻型稳定性冠心病患者80例,采用随机数表法分为对照组与治疗组,各40例。对照组给予血府逐瘀汤结合西医基础治疗,治疗组采用血府逐瘀汤联合体外反搏治疗,疗程均为30 d。评价2组治疗后的临床疗效,观察2组治疗前后中医证候积分、西雅图心绞痛量表(SAQ)积分及运动平板试验Duke评分变化;ELISA实验检测患者血清中三酰甘油(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)血脂水平及肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)、超敏C反应蛋白(hs-CRP)炎症因子水平。结果 治疗30 d后,治疗组临床总有效率(92.50%,37/40)高于对照组(80.00%,32/40)(P<0.05),2组中医证候积分较治疗前明显降低,SAQ 评分、Duke 评分均显著升高(P<0.05),治疗组优于对照组(P<0.05);患者血清中TC、TG、LDL-C水平及TNF-α、IL-6、hs-CRP水平较治疗前明显降低,HDL-C水平显著升高(P<0.05),2组治疗后HDL-C水平比较未见显著性差异(P>0.05),治疗组其余各项指标改善优于对照组(P<0.05)。结论 血府逐瘀汤联合体外反搏治疗能够进一步改善心血瘀阻型冠心病心绞痛患者的临床症状,可提高临床疗效。

Objective To observe the clinical effect of Xuefu Zhuyu Decoction combined with external counterpulsation in the treatment of angina pectoris of coronary heart disease with the pattern of stagnant blockade of heart blood.Methods A total of 80 patients with the stable coronary heart disease with the pattern of stagnant blockade of heart blood were enrolled and divided into a control group and a treatment group by the random number table method,with 40 cases in each group.The control group was given Xuefu Zhuyu Decoction combined with a basic western medicine treatment,while the treatment group was given Xuefu Zhuyu Decoction combined with external counterpulsation,both for 30 days.The clinical efficacy of the two groups after treatment was evaluated,and the changes of the TCM syndrome score,Seattle Angina Questionnaire (SAQ) score and the Duke score of treadmill exercise test were observed before and after treatment in the two groups,and the levels of the serum triacylglycerol (TG) and total cholesterol (TC),high density lipoprotein cholesterol (HDL-C),low density lipoprotein cholesterol (LDL-C),blood lipids,and the levels of the tumor necrosis factor-α (TNF-α),interleukin-6 (IL-6),hypersensitivity C-reactive protein (hs-CRP) infiammatory factors were detected by ELISA.Results After 30 days of treatment,the total clinical effective rate in the treatment group (92.5,37/40) was higher than that in the control group (80.0,32/40) (P<0.05),the TCM syndrome scores in the two groups were significantly lower than those before treatment,while the SAQ score and Duke score were significantly increased (P<0.05),and the treatment group was better than the control group (P<0.05).The levels of serum TC,TG,LDL-C and the levels of TNF-α,IL-6 and hs-CRP were significantly decreased than those before treatment,while the level of HDL-C was significantly increased (P<0.05).There was no significant difference in the level of HDL-C between the two groups after treatment (P>0.05),while the improvement of other indicators in the treatment group was better than that in the control group (P<0.05).Conclusion Xuefu Zhuyu Decoction combined with the external counterpulsation therapy can further improve the clinical symptoms of patients with angina pectoris of coronary heart disease with the pattern of stagnant blockade of heart blood,improving the clinical efficacy.
