从供给侧角度探讨基层首诊的实现对策

Implementation strategies of first consultation in primary medical institutions from the perspective of supply side

我国医疗卫生领域的供给侧结构性问题严重,倒三角的医疗卫生资源布局结构难以适应正三角的居民健康需求,阻碍了分级诊疗制度落地。分级诊疗要求的基层首诊难以实现。文章运用卫生经济学理论,从供给侧角度出发,分为供给能力和供给意愿两方面来分析困难出现的原因,提出对策。要让基层医疗卫生机构提供与分级诊疗相适应的基本医疗服务,深入推进医疗卫生领域供给侧改革,从给基层赋能、赋权、赋意愿做起。

The supply⁃side structural problems in Chinese medical and health field are serious.The“inverted triangle”medical and health resource layout structure cannot meet the needs of residents in the“regular triangle”,which hinders the implementation of the hierarchical diagnosis and treatment system.It is difficult to achieve first consultation required by hierarchical diagnosis and treatment.This paper used the theory of health economics to analyze the causes of problems and propose countermeasures from the perspective of the supply side including sup⁃ply capacity and supply willingness.If we want primary medical institutions to provide basic medical services com⁃patible with hierarchical diagnosis and treatment system,and to deepen the supply⁃side reform in the medical and health field,it is necessary to start from empowering energy,power and willingness to primary health care institu⁃tions.
