^(223)Ra注射治疗骨转移性去势抵抗性前列腺癌所致辐射影响

摘要 目的观察^(223)Ra注射治疗骨转移性去势抵抗性前列腺癌(mCRPC)对患者及周围环境的辐射影响。方法前瞻性收集9例拟接受^(223)Ra治疗的男性骨mCRPC患者,测定^(223)Ra注射后即时、30 min及1 h距注射侧肢体1 cm、30 cm、1 m及2 m处的γ射线辐射剂量率;于注射药物后第1天采集尿液样本、第1~7天分别采集粪便标本,连续测定2 min内1 ml尿液/1 g粪便中的α衰变的放射性计数总和。结果环境本底γ射线辐射剂量率为0.21μSv/h。注射药物后即时、30 min及1 h,距注射侧肢体1 cm处的γ射线辐射剂量率分别为(5.47±1.49)、(5.94±1.93)及(6.66±1.62)μSv/h,30 cm处分别为(1.27±0.23)、(1.51±0.43)及(1.46±0.51)μSv/h,1 m处分别为(0.52±0.14)、(0.54±0.14)及(0.50±0.14)μSv/h,2 m处分别为(0.23±0.04)、(0.22±0.02)及(0.22±0.02)μSv/h。环境本底放射性计数为259 counts/2 min;注射后第1天尿液放射性计数为(264.45±29.75)counts/2 min。9例中,粪便放射性计数高峰在4例见于注射后第1天、3例见于注射后第2天、2例见于注射后第3天。结论^(223)Ra注射治疗骨mCRPC患者对周围环境γ射线的辐射剂量较低,患者尿液放射性计数未见明显升高,而粪便放射性计数峰值在治疗后前3天内出现。 Objective To observe the radiation influence of^(223)Ra injection for treatment of bone metastatic castration-resistant prostate cancer(mCRPC)on patients and surrounding environment.Methods Nine male patients with bone mCRPC who would undergo^(223)Ra injection for treatment of mCRPC were prospectively enrolled.The immediately,30 min and 1 h dose rates ofγray radiation after^(223)Ra injection at 1 cm,30 cm,1 m and 2 m away from patient's injected limb were measured.Urine samples were collected at the first day after^(223)Ra injection,and fecal samples were collected at the first to the seventh days after^(223)Ra injection,and the sum of radioactivity counts ofαdecay of 1 ml urine/1 g feces was continuously measured within 2 min.Results The dose rate ofγray radiation of environmental background was 0.21μSv/h,and immediately,30 min and 1 h after^(223)Ra injection at 1 cm away from the injected limbs was(5.47±1.49),(5.94±1.93)and(6.66±1.62)μSv/h,respectively,at 30 cm away from the injected limbs was(1.27±0.23),(1.51±0.43)and(1.46±0.51)μSv/h,respectively,at 1 m was(0.52±0.14),(0.54±0.14)and(0.50±0.14)μSv/h,respectively,and at 2 m was(0.23±0.04),(0.22±0.02)and(0.22±0.02)μSv/h,respectively.The radioactivity count of environmental background was 259 counts/2 min,and of urine at the first day after^(223)Ra injection was(264.45±29.75)counts/2 min.Among 9 patients,the peak value of fecal radioactivity count was observed at the first day after^(223)Ra injection in 4 cases,at the second day in 3 cases and at the third day in 2 cases.Conclusion After^(223)Ra injection for treatment of bone mCRPC,the dose ofγray radiation of surrounding environment was low.Not obvious increase of urine radioactivity count occurred after^(223)Ra injection,and the peak value of fecal radioactivity count emerged at the first 3 days after injection.
