椎管内硬膜外脂肪增多症伴腰椎椎管狭窄1例

Spinal Epidural Lipomatosis Complicated by Lumbar Spinal Stenosis:a Case Report

目的 探讨椎管内硬膜外脂肪增多症(SEL)的临床特征、鉴别诊断及治疗方法。方法 对南昌大学第一附属医院2021年12月收治的1例SEL患者临床资料的进行回顾性分析。结果 患者女性,年龄55岁,因腰疼伴左下肢放射痛入院。术前通过问诊、查体及影像学检查诊断为SEL,行腰椎间盘及部分椎板切除椎间植骨融合内固定+椎管内脂肪去除术,疗效良好。结论 SEL临床上少见,大多临床医师对该病陌生,易误诊、漏诊。临床医师应根据患者相关病史、症状及影像学表现,判断是否合并其他疾病并排除相关鉴别诊断疾病,做出早期诊断及制定出个体化的治疗方案。

Objective To investigate the clinical characteristics,differential diagnosis and treatment of spinal epidural lipomatosis(SEL).Methods The clinical data of a SEL patient admitted to the First Affiliated Hospital of Nanchang University in December 2021 were retrospectively analyzed.Results The 55-year-old woman was admitted to the hospital due to low back pain with radiating pain in the left lower limb.The patient was diagnosed with SEL by preoperative consultation,physical examination and imaging findings,and recovered well after lumbar intervertebral disc and partial laminectomy,intervertebral bone graft fusion internal fixation and intraspinal fat removal.Conclusion SEL is a rare condition,and most clinicians are unfamiliar with this disease.Therefore,it is easy to be misdiagnosed or missed.Clinicians should judge whether there are other diseases and exclude relevant differential diagnosis according to the medical history,symptoms and imaging manifestations,so as to make early diagnosis and develop individualized treatment plans.
