基于六经辨证理论治疗视网膜分支静脉阻塞继发黄斑水肿

Treatment of macular edema secondary to branch retinal vein occlusion based on six-meridian syndrome differentiation theory

视网膜分支静脉阻塞(BRVO)是第二大常见视网膜血管性眼病,黄斑水肿(ME)是最常见的并发症,且水肿顽固难消、反复易发。中医学将其归属于“络瘀暴盲”“视瞻昏渺”的范畴,瘀血、水湿、痰浊等伴随于发病过程中,治疗多以祛瘀、利水为原则。中医眼科六经辨证理论是将中医内科学与中医眼科学相结合创立的证治体系。生理上眼与经络在循行及功能上有直接或间接的联系,病理上经络病变可影响及眼。按照六经命名,结合五轮、八廓和内眼结构对应六经说,将眼科疾病分属于六经目病。基于六经辨证理论的指导下,本文从少阴目病和太阴目病阐述BRVO-ME的发病机理及治则治法,为治疗该疾病提供有效的思维方式。

Branch retinal vein occlusion is the second most common retinal vascular opthalmopathy.Macular edema is the most common complication,which is intractable and prone to recurrence.In Traditional Chinese Medicine(TCM)theory,it belongs to the category of"collateral stasis and sudden blindness"and"blurring vision".Blood stasis,dampness and turbid phlegm are accompanied in its pathogenesis process,and the treatment is mostly based on the principle of dispelling blood stasis and promoting urination and removing dampness.The sixmeridian syndrome differentiation theory in TCM ophthalmology is a syndrome treatment system established by the combination of TCM internal medicine and TCM ophthalmology.Physiologically,there is a direct or indirect connection between the eye and the meridians in circulation and function,while pathologically,the disorders of the meridians may affect the eye.According to the six meridians,combined with the theory of five orbiculi and eight regions and inner eye structure corresponding to the six-meridian theory,the ophthalmic diseases are classified into six-meridian diseases.Under the guidance of six-meridian syndrome differentiation theory,this paper expounds the pathogenesis and treatment of macular edema secondary to branch retinal vein occlusion from Shaoyin eye disease and Taiyin eye disease,so as to provide an effective way of thinking for the treatment of this disease.
