阶梯式护理模式在头颈肿瘤患者中的应用效果

Application effect of stepwise nursing model in patients with head and neck cancer

目的探讨阶梯式护理模式应用于头颈肿瘤患者的心理护理中的临床效果。方法采用便利抽样法选取本院肿瘤科2018年3月—2019年3月收治的头颈部肿瘤患者116例,采用随机数字法分为对照组和观察组,每组各58例。对照组患者实施常规心理护理,观察组在此基础上予以阶梯式护理模式,比较两组患者干预前后心理状态及生活质量。结果干预后观察组患者医院焦虑与抑郁量表(HADS)、焦虑量表(HADS-A)、抑郁量表(HADS-D)得分均比对照组低(P<0.05);两组患者第一阶段干预成功率差异无统计学意义(P>0.05);观察组患者第二、三、四阶段及总体心理干预成功率明显高于对照组(P<0.05);干预后观察组生存质量的躯体、情绪、社会、角色、认知各领域得分均高于对照组(P<0.05)。结论阶梯式护理模式针对头颈癌患者不同程度的心理困扰,能有效降低患者负性心理,提高生活质量,值得临床推广。

Objective To explore the clinical effect of stepwise nursing model in the psychological nursing of patients with head and neck cancer.Methods 116 patients with head and neck cancer in our hospital from March 2018 to March 2019 were selected by convenience sampling method and randomly divided into control group and observation group with 58 cases in each group.The control group was given routine psychological nursing,and the observation group was given stepwise nursing model on this basis.The psychological state and quality of life of the two groups before and after intervention were compared.Result After the intervention,the scores of the Hospital Anxiety and Depression Scale(HADS),Anxiety Scale(HADS-A)and Depression Scale(HADS-D)of the observation group were lower than those of the control group(P<0.05);the first stage of the two groups There was no significant difference in the intervention success rate(P>0.05);the second,third,and fourth stages of the observation group and the overall psychological intervention success rate were significantly higher than those in the control group(P<0.05);after the intervention,the physical,emotional,and emotional aspects of the quality of life of the observation group The scores of social,role,and cognition were all higher than those of the control group(P<0.05).Conclusion The stepwise nursing model can effectively reduce the negative psychology and improve the quality of life according to different degrees of psychological distress of patients with head and neck cancer,which is worthy of clinical promotion.
