骨瓜提取物联合骨化三醇对老年骨质疏松性椎体压缩性骨折患者经皮椎体成形术后骨代谢指标的影响

Effect of Bone Melon Extract Combined with Calcitriol on Bone Metabolism Indexes after Percutaneous Vertebroplasty in Elderly Patients with Osteoporotic Vertebral Compression Fractures

目的:探讨骨瓜提取物联合骨化三醇对老年骨质疏松性椎体压缩性骨折(OVCF)患者经皮椎体成形术(PVP)后骨代谢指标的影响。方法:选取2019年1月-2021年12月收治于广州市第十二人民医院的70例OVCF患者,按随机数字表法分为对照组和观察组,各35例。所有患者均行PVP手术治疗,对照组术后给予骨化三醇,观察组在对照组的基础上联合骨瓜提取物,两组均治疗4周。比较两组疗效、视觉模拟评分法(VAS)评分、Oswestry功能障碍指数(ODI)评分、骨代谢指标[甲状旁腺激素(PTH)、25羟维生素D[25(OH)D]、骨钙素(BGP)、Ⅰ型前胶原N端肽(PⅠNP)、Ⅰ型胶原C端肽(CTX-Ⅰ)]。结果:术后4周,两组VAS评分、ODI评分均低于术前,且观察组均低于对照组,差异均有统计学(P<0.05)。术后4周,两组PTH、CTX-Ⅰ水平均低于术前,25(OH)D、BGP、PⅠNP水平均高于术前,且观察组PTH、CTX-Ⅰ水平均低于对照组,25(OH)D、BGP、PⅠNP水平均高于对照组,差异均有统计学意义(P<0.05)。观察组临床总有效率为91.43%,高于对照组的71.43%(P<0.05)。结论:骨瓜提取物联合骨化三醇对缓解老年OVCF患者行PVP后的疼痛效果显著,可改善患者腰部活动功能及骨代谢指标,疗效确切。

Objective:To investigate the effect of Bone Melon Extract combined with Calcitriol on bone metabolism indexes after percutaneous vertebroplasty(PVP)in elderly patients with osteoporotic vertebral compression fractures(OVCF).Method:A total of 70 OVCF patients admitted to the Twelfth People’s Hospital of Guangzhou from January 2019 to December 2021 were selected,they were divided into the control group and the observation group according to the random number table method,with 35 cases in each group.All patients were treated with PVP operation.The control group was given Calcitriol after operation,and the observation group was combined with Bone Melon Extract on the basis of the control group.Both groups were treated for 4 weeks.The efficacy,visual analogue scale(VAS)scores,Oswestry disability index(ODI)scores,bone metabolic indexes[parathyroid hormone(PTH),25-hydroxyvitamin D[25(OH)D],osteocalcin(BGP),procollagen typeⅠN-terminal peptide(PⅠNP),collagen typeⅠC-terminal peptide(CTX-Ⅰ)]were compared between two groups.Result:At 4 weeks after operation,VAS scores and ODI scores of both groups were lower than those before operation,and those of the observation group were lower than those of the control group,and the differences were statistically significant(P<0.05).At 4 weeks after operation,the levels of PTH and CTX-Ⅰof both groups were lower than those before operation,the levels of 25(OH)D,BGP and PⅠNP of both groups were higher than those before operation,and the levels of PTH and CTX-Ⅰof the observation group were lower than those of the control group,and the levels of 25(OH)D,BGP and PⅠNP of the observation group were higher than those of the control group,the differences were statistically significant(P<0.05).The total effective rate of the observation group was 91.43%,which was higher than 71.43%of the control group(P<0.05).Conclusion:Bone Melon Extract combined with Calcitriol has significant effect on alleviating the pain of elderly OVCF patients after PVP,and can improve the waist function and bone metabolism indexes of the patients,and the effect is definite.
