摘要
目的:探究磁共振波谱成像(MRS)在评价脑白质疏松症患者认知功能损害中的应用价值。方法:将2020年6月至2021年5月广东省茂名市中医院收治的50例脑白质疏松症患者作为观察组,另招募50例健康志愿者作为对照组。采集两组研究对象的MRS相关参数进行对比,分析MRS参数与脑白质疏松症患者认知功能之间的相关性。结果:观察组的简易智能精神状态检查量表(MMSE)评分和蒙特利尔认知评估量表(MoCA)评分均低于对照组健康人(P<0.05)。在MRS参数方面,观察组患者的N-乙酰天门冬氨酸(NAA)/胆碱(Cho)和NAA/肌酸(Cr)均低于对照组健康人(P<0.05);两组研究对象的Cho/Cr无明显差异(P>0.05)。经person相关性分析得知,观察组患者的MRS参数NAA/Cr与其MMSE评分存在显著的正相关(r=0.44,P<0.05)。结论:脑白质疏松症患者MRS中部分代谢产物的相对表达量与健康人群有显著差异,且MRS参数与此病患者认知功能损害的程度成正相关。MRS在评价脑白质疏松症患者的认知功能损害中具有较高的应用价值。
Objective:To explore the application value of magnetic resonance spectroscopy(MRS)in the evaluation of cognitive impairment in patients with leukoaraiosis.Methods:A total of 50 patients with leukoaraiosis admitted to Maoming Hospital of Traditional Chinese Medicine,Guangdong Province from June 2020 to May 2021 were selected as the observation group,and 50 healthy volunteers were recruited as the control group.The MRS related parameters of the two groups were collected and compared,and the correlation between MRS parameters and cognitive function of leukoaraiosis patients was analyzed.Results:The scores of Mini-Mental State Examination(MMSE)and Montreal Cognitive Assessment(MoCA)in the observation group were lower than those in the control group(P<0.05).In terms of MRS parameters,N-acetylaspartate(NAA)/choline(Cho)and NAA/creatine(Cr)in the observation group were lower than those in the control group(P<0.05).There was no significant difference in Cho/Cr between the two groups(P>0.05).According to Person correlation analysis,there was a significant positive correlation between MRS parameter NAA/Cr and MMSE score in the observation group(r=0.44,P<0.05).Conclusion:The relative expression levels of some metabolites in MRS of patients with leukoaraiosis are significantly different from those of healthy people,and MRS parameters are positively correlated with the degree of cognitive impairment in patients with leukoaraiosis.MRS has a high application value in the evaluation of cognitive impairm ent in patients with leukoaraiosis.
