大承气汤对胆囊结石腹腔镜手术患者术后炎症因子水平及胃肠功能恢复的影响

Effect of Dachengqi Decoction on Postoperative Inflammatory Factors Levels and Gastrointestinal Function Recovery in Patients Undergoing Laparoscopic Surgery for Gallstones

目的探讨胆囊结石腹腔镜手术患者术后经大承气汤治疗的临床效果。方法选择江西省玉山县中医院2018年8月-2020年9月收治的60例胆囊结石患者,采用随机数字表法分为2组,每组30例。所有患者均行腹腔镜胆囊切除术,对照组术后给予常规治疗,观察组术后给予大承气汤治疗。对比2组炎症因子水平、胃肠功能恢复情况及不良反应发生情况。结果观察组术后IL-6、TNF-α、CRP水平低于对照组,观察组肛门排气时间、肠鸣音恢复时间、排便时间短于对照组,观察组不良反应发生率为6.67%(2/30),低于对照组的26.67%(8/30),差异有统计学意义(P<0.05)。结论胆囊结石腹腔镜手术患者术后经大承气汤治疗对减轻炎症反应,加速胃肠功能恢复具有积极的作用,且安全可靠,是一种理想的治疗方案。

Objective To investigate the clinical effect of Dachengqi decoction for postoperative treatment of gallstone patients undergoing laparoscopic surgery.Methods A total of 60 patients with gallstones admitted to Yushan County Hospital of Traditional Chinese Medicine from August 2018 to September 2020 were selected and divided into two groups by random number table method,with 30 patients in each group.All patients received laparoscopic cholecystectomy,the control group was given conventional treatment after surgery,and the observation group was given Dachengqi decoction after surgery.The levels of inflammatory factors,the recovery of gastrointestinal function and the incidence of adverse reactions were compared between the two groups.Results The levels of IL-6,TNF-αand CRP in the observation group were lower than those in the control group.The time of anal exhaust,bowel sounds recovery and defecation in the observation group were shorter than those in the control group.The incidence of adverse reactions in the observation group was 6.67%(2/30),lower than 26.67%(8/30)in the control group.The difference was statistically significant(P<0.05).Conclusion Dachengqi decoction has positive effect on reducing inflammatory reaction and accelerating gastrointestinal function recovery in patients undergoing laparoscopic cholecystolithiasis operation.It is safe and reliable,and is an ideal treatment plan.
