通络解痉汤联合强化降脂抗凝药对脑卒中恢复期患者临床疗效及血液流变学指标的影响

Effect of Tongluo Jiejing Decoction combined with enhanced lipid-lowering anticoagulant on clinical efficacy and hemorheology of patients with convalescent stroke

目的观察通络解痉汤联合强化降脂抗凝药对脑卒中恢复期患者临床疗效及血液流变学指标的影响。方法将我院收治的112例脑卒中恢复期患者按照治疗方式分为对照组与观察组各56例。两组患者入组后均给予常规基础治疗,对照组给予强化降脂抗凝药质量,观察组在对照组治疗基础上给予通络解痉汤治疗,对比两组患者临床疗效及治疗前后简易精神状态检查量表(MMSE)、蒙特利尔认知评估量表(MoCA)、日常生活能力量表(ADL)、Fugl-Meyer运动功能评定量表(FMA)、卒中量表(NIHSS)评分及患者大脑前动脉(ACA)、中动脉(MCA)及后动脉(PCA)的平均血流速度(Vm)。结果治疗后,观察组患者临床治疗总有效率显著高于对照组(P<0.05);两组NIHSS评分均显著降低(P<0.05),FMA、ADL评分均显著增加(P<0.05),且观察组NIHSS评分显著低于对照组(P<0.05),FMA、ADL评分均显著高于对照组(P<0.05);两组MMSE、MoCA量表评分显著高于治疗前(P<0.05),观察组MMSE、MoCA量表评分均显著高于对照组(P<0.05);两组ACA、MCA、PCA的Vm均显著高于治疗前(P<0.05),且观察组ACA、MCA、PCA的Vm均显著高于对照组(P<0.05)。结论通络解痉汤配合强化降脂抗凝药治疗脑卒中恢复期患者,能够有效改善患者的认知功能和肢体运动功能,改善颅内血流动力学状态,提高日常生活能力,疗效显著,值得临床推广。

Objective To observe the Effect of Tongluo Jiejing Decoction combined with enhanced lipid-lowering anticoagulant on clinical efficacy and hemorheology of patients with convalescent stroke.Methods A total of 112 patients with convalescent stroke were divided into control group and observation group according to treating methods,each with 56 cases.Both groups were treated with routine basic teratment.Control group was given anti-coagulant;observation group was given Tongluo Jiejing Decoction on the basis of control group.The clinical efficacy and Simple Mental State Examination(MMSE),Montreal Cognitive Assessment Scale(MoCA),Daily Life Ability Scale(ADL),Fugl-MeyerMotor Function Assessment Scale(FMA),National Institute of Health Stroke Scale(NIHSS)and mean blood flow velocity(Vm)of anterior cerebral artery(ACA),middle artery(MCA)and posterior artery(PCA)were compared between groups before and after treatment.Results After treatment,the total effective rate in observation group was higher than in control group(P<0.05);the NIHSS scores in both groups decreased significantly(P<0.05),the FMA,ADL scores increased significantly(P<0.05);and the NIHSS scores in observation group were significantly lower than in control group(P<0.05),the FMA,ADL scores were significantly higher than control group(P<0.05);the MMSE,MoCA scores were significantly higher than that before treatment in both groups(P<0.05),which were significantly higher in observation group than in control group(P<0.05);the Vm of ACA,MCA,PCA were significantly higher than that before treatment in both groups(P<0.05),and were significantly higher in observation group than in control group(P<0.05).Conclusion Tongluo Jiejing Decoction combined with enhanced lipid-lowering anticoagulant can effectively improve the cognitive function and limb motor function,improve the intracranial hemodynamic state,improve the ability of daily life in treatment convalescent stroke.The curative effect is remarkable,and worthy of clinical promotion.
