入睡困难型失眠症病因病机及辨证论治探讨

Discussion on etiology,pathogenesis,syndrome differentiation and treatment of insomnia with difficulty falling asleep

中医对入睡困难型失眠症有独特的认识,若对本病的病因病机、辨证论治等方面进行归纳,可为不同睡眠主症失眠症的中医诊治提供思路。目前中医认为入睡困难型失眠症的病因病机多为阴阳失和、体虚、脏腑功能失调以及情志失调等,中医分型尚未统一,主要可见肝郁化火、阴虚火旺、心胆气虚、心脾两虚以及心肾不交五型。在治疗方面,可从不同角度入手,使用经典汤药、自拟方、中成药以及中医外治法皆能有效缩短患者入睡时间,并改善其睡眠质量。

Traditional Chinese medicine(TCM)has a unique understanding of the etiology and pathogenesis of insomnia with difficulty in falling asleep. If the etiology,pathogenesis,syndrome differentiation and treatment of this disease are summarized,it can provide ideas for the TCM diagnosis and treatment of insomnia. At present,the views on this disease are summarized as yin-yang disharmony,body deficiency,visceral dysfunction and emotional disorder. The classification of TCM has not been unified. At present,insomnia with difficulty in falling asleep is mainly divided into five types:liver fire,Yin deficiency and Fire exuberance,qi deficiencies of the heart and gallbladder,deficiency of heart and spleen and disharmony between heart and kidney. In terms of treatment,we can start from different angles. Classical decoctions,self-made formulas,Chinese patent medicines and external treatments of TCM,all of them can effectively shorten the time for patients to fall asleep and improve their sleep quality. This paper summarized the understanding and research progress of insomnia with difficulty in falling asleep in TCM in recent years,and summarizes the causes,syndromes and treatment of TCM.
