针刺联合鼻炎喷剂治疗儿童变应性鼻炎临床疗效及其安全性观察

Clinical curative effect and safety of acupuncture combined with external rhinitis spray on children with allergic rhinitis

目的观察针刺联合外用鼻炎喷剂治疗儿童变应性鼻炎(AR)临床疗效及其安全性。方法将我院收治的104例AR患儿随机分为联合组与对照组各52例。对照组予鼻炎喷剂外用治疗,联合组在对照组治疗基础上联合针刺治疗,连续治疗3周后评估临床疗效,观察两组AR症状体征评分、血清总免疫球蛋白E(IgE)、螨特异性IgE、白细胞介素-4(IL-4)、白细胞介素-10(IL-10)水平在治疗前后的变化。结果治疗后,联合组总有效率显著高于对照组(P<0.05);两组患儿的总IgE水平无变化(P>0.05);但特异性IgE水平、各类症状体征记分、血清IL-4、IL-10水平均下降(P<0.05),且联合组下降更明显(P<0.05);两组在针刺或用药期间未出现血、尿、便常规及肝肾指标异常的情况,仅少数患儿出现鼻出血、鼻灼热或刺激、口鼻干燥等,两组不良反应总发生率比较无明显差异(P>0.05)。结论针刺联合外用鼻炎喷剂治疗AR患儿效果明显,能调节患儿机体免疫状态、炎性因子水平等,对于症状体征改善方面更有优势,且安全可靠。

Objective To observe the clinical curative effect and safety of acupuncture combined with exterhal rhinitis spray on children with allergic rhinitis(AR).Methods A total of 104 AR children were randomly divided into combination group and control group,52 cases in each group.Control group was treated with rhinitis spray externally,while combination group was treated with acupuncture on basis of control group.After 3 weeks of continuous treatment,the clinical curative effect was evaluated;the changes in scores of AR symptoms and signs,levels of serum total IgE,mite-specific IgE,interleukin(IL)-4 and IL-10 before and after treatment were compared between the two groups.Results After treatment,the total effective rate was significantly higher in combined group than in control group(P<0.05);the total IgE level of both groups had no change(P>0.05),and the specific IgE level,the scores of various symptoms and signs,the serum levels of IL-4 and IL-10 decreased significantly(P<0.05),which decreased more obviously in combination group than in control group;there was no abnormality of blood,urine,stool routine and liver and kidney indexes in the two groups during acupuncture or medication,only a few patients had epistaxis,nasal burning or irritation,dry mouth and nose,etc.,there was no significant difference in total incidence of adverse reactions between the two groups(P>0.05).Conclusion The curative effect of acupuncture combined with exterhal rhinitis spray is significant on children which AR.It can regulate immune status and levels of inflammatory factors,it is advantageous in improving symptoms and signs,and is safe and reliable.
