下肢肌筋膜触发点温针灸疗法对帕金森下肢僵硬改善平衡功能的影响

Study on the improvement of balance function of Parkinsonian lower extremity stiffness with thermal acupuncture at lower extremity myofascial trigger points

目的观察下肢肌筋膜触发点温针灸疗法对帕金森下肢僵硬患者平衡功能的改善作用。方法将我院100例帕金森下肢僵硬患者按治疗方式分为观察组与对照组各50例。对照组采用运动疗法,观察组采用运动和下肢肌筋膜触发点温针灸疗法。观察两组患者临床疗效,比较治疗前后两组患者统一帕金森病评定量表第Ⅱ部分(UPDRSⅡ)、UPDRSⅢ、6min步行距离(6MWD),简化Fugl-Meyer运动功能评分(FMA)、Berg平衡量表(BBS)、修订版跌倒效能量表(MFES)、步长、步频、步速。结果治疗后,观察组患者总有效率显著高于对照组患者(P<0.05);两组患者UPDRSⅡ、UPDRSⅢ、步频下降,6MWD、FMA、BBS、MFES得分、步长、步速增加,观察组UPDRSⅡ、UPDRSⅢ、步频低于对照组,6MWD、FMA、BBS、MFES得分、步长、步速高于对照组(P<0.05)。结论下肢肌筋膜触发点温针灸疗法提高帕金森下肢僵硬患者运动功能和日常活动能力,增强平衡和步行能力,改善患者害怕跌倒的状况,临床疗效显著。

Objective To observe the effect of thermal acupuncture on lower extremity myofascial trigger points on the balancing function of patients with Parkinsonian lower extremity stiffness.Methods A hundred patients with Parkinsonian lower extremity stiffness were divided into observation group and control group,each 50 cases.Control group received exercise therapy,while observation group received exercise and lower limb myofascial trigger point thermal acupuncture.The clinical efficacy of the two groups was observed.The Parkinson’s Disease Rating Scale PartⅡ(UPDRSⅡ),UPDRSⅢ,6-min walking distance(6MWD),Simplified Fugl-Meyer Motor Function(FMA),and Berg Balance Scale(BBS),Modified Fall Efficacy Scale(MFES)scores,stride length,cadence,pace before and after treatment were compared between the two groups.Results After treatment,the total effective ratewas significantly higher in observation group than in control group(P<0.05);UPDRSⅡ,UPDRSⅢ,and stride frequency decreased;6MWD,FMA,BBS,MFES scores,stride length,and stride speed increased in both groups.UPDRSⅡ,UPDRSⅢ,and stride frequency were lower in observation group,and step length and pace scores were higher than in control group(P<0.05).Conclusion The lower limb myofascial trigger point thermal acupuncture can improve the motor function and daily activity ability of patients with Parkinsonian lower limb stiffness,enhance the balancing and walking ability,and release the fear of falling,and the clinical effect is significant.
