突发性耳聋与血脂、免疫球蛋白水平以及凝血功能的相关性分析

Correlation between sudden deafness and blood lipid profile, immunoglobulin levels, and coagulation function

目的 探讨突发性耳聋(简称突聋)与生化指标(凝血、血脂和免疫)的相关性。方法 选择2014年1月~2020年10月来我院耳鼻喉科就诊的突聋患者标本979例(突聋组),975例健康标本作为对照(健康对照组)。测定所有研究对象的凝血指标[活化部分凝血活酶时间(APTT)、凝血酶原时间国际标准化比值(INR)、凝血酶时间(TT)、纤维蛋白原(FIB)、D-二聚体(D-D)],血脂指标[总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、载脂蛋白A(ApoA)、载脂蛋白B(ApoB)]和免疫指标[免疫球蛋白M(IgM)、IgG、IgA、C3C、C4)],并进行相关性分析。结果 突聋组的APTT、INR、TT、FIB和D-D水平分别为31.89 s、1.01、19.43 s、2.28 g/L和3.96 mg/L,对照组为31.63 s、1.02、19.48 s、2.41g/L和0.29 mg/L,其中FIB和D-D在2组之间的差异具有统计学意义;凝血指标与性别无关,而与年龄关系密切。突聋患者的TC、TG和LDL-C水平分别为4.90、1.66、3.25 mmol/L,高于对照组相应指标水平。突聋组IgG含量明显低于对照组(P<0.001)。结论 突聋的发生与患者凝血功能、血脂水平以及免疫水平异常存在关联。

Objective To analyze the correlations between biochemical indicators such as coagulation,lipid and immunity and sudden deafness(SD). Methods Nine hundred and seventy-nine samples from patients with sudden deafness who came to our hospital from January 2014 to October 2020 were selected as SD group,while 975 samples from healthy people were selected as the control. The coagulation indexes including activated partial thromboplastin time(APTT), international normalized ratio of prothrombin time(INR), thrombin time(TT), fibrinogen(FIB) and D-Dimer(D-D), blood lipid indexes including total cholesterol(TC), triglycerides(TG), low density lipoprotein cholesterol(LDL-C), high density lipoprotein cholesterol(HDL-C), apolipoprotein A(Apo A), and apolipoprotein B(ApoB), immune indexes including immunoglobulin M(IgM), IgG, IgA, C3C, C4were detected and analyzed statistically. Results APTT, INR, TT, FIB and D-D levels of the coagulation index in the SD group were 31.89 s, 1.01, 19.43 s, 2.28 g/L, 3.96 mg/L, respectively and the corresponding indexes in the control group were 31.63 s, 1.02, 19.48 s, 2.41 g/L, 0.29 mg/L. The differences in FIB and D-D were statistically significant between the two groups. The coagulation indexes were not related to the gender, while were related to the age. The levels of TC, TG and LDL-C in SD group were 4.90 mmol/L, 1.66 mmol/L, and 3.25 mmol/L, which were higher than the corresponding indexes in the control group. The immunological index IgG level in SD group was significantly lower than that in the control group. Conclusions The occurrence of SD is associated with abnormal blood coagulation, lipid metabolism and immune level.
