醒脑解痉针法联合平衡针治疗卒中后下肢肌张力增高临床研究

Clinical Study of Xingnao Jiejing Needling Combined with Balance Acupuncture in Treatment of Increased Muscle Tone of Lower Extremity after Stroke

目的:探讨醒脑解痉针法联合平衡针治疗卒中后下肢肌张力增高的临床疗效。方法:选取2019年6月—2020年12月本院收治的卒中后下肢肌张力增高患者95例,采用随机数表法分为对照组48例和观察组47例,对照组给予平衡针疗法,观察组在对照组基础上给予醒脑解痉针法,比较两组治疗后临床疗效、治疗前后FMA评分、Barthel指数评分、下肢肌张力评分、血流动力学指标及血清IL-6、TNF-α和hs-CRP水平变化。结果:治疗后两组FMA评分、Barthel指数评分和下肢肌张力评分较治疗前均明显升高,差异具有统计学意义(P<0.05),且观察组FMA评分、Barthel指数评分和下肢肌张力评分与对照组比较升高更显著,差异具有统计学意义(P<0.05);两组治疗后Vmax、Vm和BF值较治疗前显著升高,差异具有统计学意义(P<0.05),且观察组Vmax、Vm和BF值与对照组比较升高更显著,差异具有统计学意义(P<0.05);治疗后两组血清IL-6、TNF-α和hs-CRP水平较治疗前显著降低,差异具有统计学意义(P<0.05),且观察组血清IL-6、TNF-α和hs-CRP水平与对照组比较降低更显著,差异具有统计学意义(P<0.05);观察组治疗总有效率显著高于对照组,差异具有统计学意义(P<0.05)。结论:醒脑解痉针法联合平衡针治疗卒中后下肢肌张力增高疗效较好,且能够改善下肢血流动力学指标。

Objective:To explore the clinical study of Xingnao Kaiqiao needling combined with balance acupuncture in the treatment of increased muscle tone of lower extremity muscle after stroke.Methods:A total of 95 patients with increased muscle tone of lower extremity after stroke were randomly divided into the control group(n=48)and the observation group(n=47).The control group was treated with balance acupuncture;on which basis,the observation group was also treated with Xingnao Jiejing needling.Clinical efficacy was compared between the two groups after the treatment.FMA score,Barthel index score,lower limb muscle tension score,hemodynamic indexes and serum levels of IL-6,TNF-αand hs-CRP were compared between before and after the treatment in the two groups.Results:FMA score,Barthel index score and lower limb muscle tension score were significantly higher after the treatment than those before the treatment in the two groups(P<0.05);of which the increases were more significant in the observation group than those in the control group(P<0.05).The values of Vmax,Vm and BF were significantly increased after the treatment compared with those before the treatment in the two groups(P<0.05);of which the increases were more significant in the observation group than those in the control group(P<0.05).The serum levels of IL-6,TNF-αand hs-CRP were significantly decreased after the treatment compared with those before the treatment in the two groups(P<0.05);of which the decreases were more significant in the observation group than those in the control group(P<0.05).The effective rate of observation group was significantly higher than that of the control group(P<0.05).Conclusion:Xingnao Jiejing needling combined with balance acupuncture is effective in the treatment of increased muscle tone of lower extremity after stroke,and it can improve hemodynamic indexes of lower extremity.
