超声造影在甲状腺结节微波消融术中的临床应用 被引量:2

摘要 目的探讨甲状腺结节微波消融术中超声造影的临床应用价值。方法选取某院2021年5—11月行微波消融治疗术的甲状腺良性结节患者78例作为研究对象,随机将其分为研究组和对照组各39例。研究组应用超声造影监控指导,对照组应用常规超声监控指导。比较两组患者手术前后甲状腺激素水平、消融灶体积、体积缩小率以及甲状腺结节完全消融率和结节复发率。结果研究组术后6个月TSH低于对照组,FT、FT高于对照组;研究组术后6个月、12个月及18个月体积明显较对照组小,体积缩小率明显较对照组高,差异具有统计学意义(P<0.05);研究组术后18个月完全消融率明显较对照组高,结节复发率低于对照组,差异有统计学意义(P<0.05)。本研究表明,消融治疗中应用超声造影监控指导,较常规超声能够更准确有效的监控甲状腺结节的消融情况。结论超声造影用于甲状腺结节微波消融术较常规超声能够更准确的评估消融范围,提高完全消融率,降低结节术后复发率,临床应用价值较高。 Objective To explore the clinical application value of contrast-enhanced ultrasound in microwave ablation treatment of thyroid nodules.Methods Seventy-eight patients with benign thyroid nodules who were treated with microwave ablation from May to November 2021 in one hospital were selected as the research objects.They were randomly assigned to study group and control group with 39 cases each.The study group was guided by contrast-enhanced ultrasound,and the control group was guided by conventional ultrasound.The thyroid hormone levels,ablation lesion volume,volume reduction rate,total ablation rate of thyroid nodules and the recurrence rate of nodules between the two groups was compared before and after the surgery.Results The level of FSH was lower than the control group,the level of Tand Twas higher than the control group 6 months after the surgery.The volume of nodules was significantly smaller,and the volume reduction rate was significantly higher than the control group at 6 months,12 months,and 18 months after the surgery,and the difference was statistically significant(P<0.05);The total ablation rate of the study group was significantly higher than the control group at 18 months after surgery,and the recurrence rate of nodules was lower than the control group,and the difference was statistically significant(P<0.05).The study showed that the application of contrast-enhanced ultrasound in thyroid nodules ablation treatment could monitor the state of ablation more accurately and effectively than conventional ultrasound.Conclusion Contrast-enhanced ultrasound can assess the ablation range more accurately than conventional ultrasound.The use of contrast-enhanced ultrasound in microwave ablation can improve the total ablation rate of thyroid nodules,can also reduce the recurrence rate of nodules.It has high clinical application value.
