Comparison on Effects and Adverse Reactions Between Transfusion Machine-Collected Platelets and Irradiation Machine-Collected Platelets in Children

摘要 目的探讨儿童输注机采血小板与辐照机采血小板的效果,比较二者不良反应。方法回顾性分析2020年1月至2021年12月开封市某医院120例行血小板输注的血液病患儿的临床资料。根据患儿输注的血小板制备方式分为A组和B组,将其中59例输注辐照机采血小板的患儿的临床资料纳入A组,将61例输注机采血小板的患儿的临床资料纳入B组。比较两组输注血小板前、输注血小板后24 h的外周血血小板计数(PLT);比较两组患儿输注血小板后24 h的临床疗效[采用校正血小板计数增高指数(CCI)和血小板回收率(PPR)评估];统计两组血小板输注期间及输注后72 h内不良反应发生情况。结果血小板输注后24 h,两组PLT均较输注血小板前上升(P<0.05),两组PLT水平比较,差异无统计学意义(P>0.05)。两组基于CCI和PPR测定的血小板输注后24 h的有效率相近,差异无统计学意义(P>0.05)。A组血小板输注期间及输注后72 h内不良反应发生率低于B组(P<0.05)。结论输注辐照机采血小板与机采血小板对血液病患儿效果相当,但前者不良反应发生率更低。 Objective To investigate the effects of transfusion machine-collected platelets and irradiation machine-collected platelets in children,and to compare the occurrence of adverse reactions between them.Methods The clinical data of 120 children with hematological diseases who underwent platelet transfusion in a hospital of Kaifeng from January 2020 to December 2021 were retrospectively analyzed.According to the preparation methods of platelets,the children were divided into group A and group B.The clinical data of 59 children who were infused with irradiation machine-collected platelets were included in group A,and the clinical data of 61 children who were infused with transfusion machine-collected platelets were included in group B.The peripheral blood platelet count(PLT)before platelet transfusion and 24 hours after platelet transfusion were compared between the two groups.The clinical effects of 24 hours after platelet transfusion were compared between the two groups[evaluated by corrected platelet count index(CCI)and platelet recovery rate(PPR)].The adverse reactions during platelet transfusion and within 72 hours after transfusion were counted.Results At 24 hours after platelet transfusion,the level of serum PLT in both groups was increased compared with that before platelet transfusion(P<0.05),but there was no statistical significant difference in PLT level between the two groups(P>0.05).The effective rates of both groups 24 hours after platelet infusion based on CCI and PPR were similar,and there was no statistical significant difference(P>0.05).The incidence of adverse reactions during and 72 hours after platelet infusion in group A was lower than that in group B(P<0.05).Conclusion The effect of irradiation machine-collected platelets infusion is equivalent to that of machine-collected platelets in children with hematological diseases,but the incidence of adverse reactions in the former is lower.
