摘要
目的探析急性呼吸衰竭患者经无创呼吸机治疗效果的影响因素。方法回顾性分析焦作市人民医院2018年4月至2021年1月收治的240例急性呼吸衰竭患者的临床资料,分为治疗无效组(41例)和治疗有效组(199例),采用logistic回归分析急性呼吸衰竭患者治疗效果的影响因素。结果治疗无效组患者调节性T(Treg)细胞占比低于治疗有效组,差异有统计学意义(P<0.05);治疗无效组和治疗有效组意识障碍、患者-呼吸机同步情况以及肺部感染情况比较,差异有统计学意义(P<0.05);logistic回归分析结果显示,意识障碍、患者-呼吸机不同步、肺部感染、Treg细胞占比低是急性呼吸衰竭患者经无创呼吸机治疗无效的影响因素(OR>1,P<0.05)。结论意识障碍、患者-呼吸机不同步、肺部感染、Treg细胞占比低是急性呼吸衰竭患者经无创呼吸机治疗无效的影响因素。
Objective To explore the influencing factors of efficacy of non-invasive ventilator treatment in patients with acute respiratory failure.Methods The clinical data of 240 patients with acute respiratory failure admitted to Jiaozuo People’s Hospital from April 2018 to January 2021 were retrospectively analyzed.They were divided into treatment ineffective group(41 cases)and treatment effective group(199 cases).Logistic regression was used to analyze the influencing factors of treatment effect in patients with acute respiratory failure.Results The percentage of regulatory T(Treg)cells in the ineffective group was lower than that in the effective group(P<0.05).There was significant difference in consciousness disorder,patient ventilator synchronization and pulmonary infection between the ineffective group and the ineffective group(P<0.05).Logistic regression analysis showed that consciousness disturbance,patient ventilator asynchrony,lung infection and low proportion of Treg cells were the influencing factors of ineffective treatment with non-invasive ventilator in patients with acute respiratory failure(OR>1,P<0.05).Conclusion Consciousness disorder,patient ventilator asynchrony,lung infection and low proportion of Treg cells are the influencing factors of ineffective treatment with non-invasive ventilator in patients with acute respiratory failure.
