急诊脑梗死患者溶栓治疗后发生脑水肿的相关因素

Related Factors of Cerebral Edema After Thrombolytic Therapy in Patients with Acute Cerebral Infarction

目的分析急诊脑梗死患者溶栓治疗后发生脑水肿的影响因素。方法选取2018年1月至2021年5月于商丘市第一人民医院接受溶栓治疗的70例急诊脑梗死患者作为研究对象,患者溶栓治疗后均接受3个月的随访。观察并统计患者溶栓后脑水肿发生情况并分组,整理患者基线资料,采用logistic回归分析急诊脑梗死患者溶栓治疗后发生脑水肿的影响因素。结果患者术后均获得3个月随访结果,70例患者中30例(42.86%)发生脑水肿;发生组脑梗死面积、合并糖尿病例数、白细胞计数与未发生组比较,差异有统计学意义(P<0.05);经logistic回归结果显示,脑梗死面积大、合并糖尿病、白细胞计数高是急性脑梗死溶栓治疗后患者脑积水发生的影响因素(OR>1,P<0.05)。结论脑梗死面积大、合并糖尿病、白细胞计数影响急诊脑梗死患者溶栓治疗后脑水肿的发生。

Objective To analyze the influencing factors of brain edema after thrombolytic therapy in patients with acute cerebral infarction.Methods A total of 70 patients with acute cerebral infarction who received thrombolytic therapy in the First People’s Hospital of Shangqiu from January 2018 to May 2021 were selected as the research subjects.All patients were followed up for 3 months after thrombolytic therapy.The occurrence of brain edema after thrombolysis in patients were observed and counted,and divided them into groups.The baseline data of patients were sorted out,and the influencing factors of brain edema after thrombolysis in patients with acute cerebral infarction by logistic regression were analyzed.Results All patients were followed up for 3 months,and 30 of 70 patients(42.86%)developed brain edema.The area of cerebral infarction,the number of cases with diabetes and the white blood cell count in the occurrence group were statistically different from those in the non occurrence group(P<0.05).Logistic regression showed that large cerebral infarction area,combined with diabetes and high white blood cell count were the influencing factors of hydrocephalus in patients with acute cerebral infarction after thrombolytic therapy(OR>1,P<0.05).Conclusion The occurrence of cerebral edema after thrombolytic therapy in patients with acute cerebral infarction is influenced by the large area of cerebral infarction,diabetes mellitus and leukocyte counts.
