血清相关因子水平与急性缺血性脑卒中患者阿替普酶静脉溶栓后预后的相关性

Correlation Between Serum Related Factors and Prognosis of Acute Ischemic Stroke Patients After Intravenous Thrombolysis with Recombinant Tissue Plasminogen Activator

目的分析血清相关因子水平与急性缺血性脑卒中(AIS)患者阿替普酶(rt-PA)静脉溶栓后预后的相关性。方法选取淅川县人民医院2019年10月至2021年12月收治的238例AIS患者作为疾病组,并选取同期健康体检者159例作为健康对照组,于rt-PA静脉溶栓治疗7 d后根据美国国立卫生研究院卒中量表(NIHSS)评分将疾病组分为预后良好组(142例)、预后不良组(96例)。采集所有入选者血液,测定血清尿酸(UA)、成纤维细胞生长因子4(FGF4)、脑钠肽(BNP)、过氧化还原蛋白1(PRDX1)水平,分别比较疾病组与健康对照组、预后良好组与预后不良组上述血清因子水平,分析其与预后的关联性。结果疾病组血清UA、BNP、FGF4、PRDX1水平均较健康对照组高(P<0.05),预后不良组血清UA、BNP、FGF4、PRDX1水平较预后良好组高(P<0.05)。相关性分析显示,AIS患者血清UA、BNP、FGF4、PRDX1水平与rt-PA静脉溶栓后预后不良呈正相关(P<0.05)。结论血清UA、BNP、FGF4、PRDX1水平在AIS患者中呈异常表达,且与临床预后紧密相关。

Objective To analyze the correlation between serum related factors and prognosis of acute ischemic stroke(AIS)patients after recombinant tissue plasminogen activator(rt-PA).Methods Two hundred and thirty-eight patients with AIS in Xichuan County People’s Hospital from October 2019 to December 2021 were selected as disease group,and 159 healthy check-ups were selected as healthy controls during the same period.Sevens days after rt-PA intravenous thrombolysis treatment,the disease group was divided into good prognosis group(142 cases)and poor prognosis group(96 cases)according to the National Institutes of health stroke scale(NIHSS)score.The levels of serum uric acid(UA),fibroblast growth factor 4(FGF4),brain natriuretic peptide(BNP),and peroxiredoxin 1(PRDX1)were measured in the blood of all participants.The correlations between the levels of these factors were analyzed in the disease group and the healthy control group,and in the good prognosis group and the poor prognosis group.Results Serum UA,BNP,FGF4,and PRDX1 levels were higher in the disease group than those in the healthy control group(P<0.05).Serum UA,BNP,FGF4,and PRDX1 levels were higher in the poor prognosis group than those in the good prognosis group(P<0.05).Correlation analysis revealed that serum UA,BNP,FGF4,and PRDX1 levels of AIS patients were positively correlated with poor prognosis after rt-PA intravenous thrombolysis(P<0.05).Conclusion Serum UA,BNP,FGF4,and PRDX1 levels were abnormally expressed in patients with AIS and were strongly correlated with clinical prognosis.
