儿科抗菌药物超药品使用说明书使用现况调查

Investigation on the Use of Pediatric Antibacterial Drugs Beyond the Instructions for Use

目的分析儿科抗菌药物超药品使用说明书使用现况,以为临床规范、安全用药合理化建议提供可靠依据。方法选取郸城县人民医院2021年1月至2022年5月含抗菌药物处方486份进行研究,分析不合理应用现况。结果抗菌药物包括口服剂型350例(以头孢克洛为主,占比23.66%)与静脉给药剂型235例(以头孢呋辛钠为主,占比18.72%)。含抗菌药物处方用药涉及疾病类型较多,主要为呼吸系统感染,占比为82.71%。含抗菌药物处方内单一用药369例[236例单一口服(48.56%),133例单一静脉(27.37%)],二联用药主要为序贯用药,以静脉用药头孢呋辛钠、口服头孢呋辛酯为主,占比11.11%。抗菌药物处方中合理用药为334例,占比68.72%,不合理用药为152例,占比31.28%;用药不合理原因以无指征用药为主,其次为不合理的用法用量与药物选择。结论儿科抗菌药物存在应用不合理状况,以无指征用药、用法用量不合理及药物选择不合理等为主。

Objective To analyze the current situation of the use of pediatric antibacterial drugs beyond the instruction manual,in order to provide a reliable basis for clinical standardization and rationalization of safe drug use.Methods A total of 486 prescriptions containing antibacterial drugs from January 2021 to May 2022 in Dancheng County People’s Hospital were selected for research,and the current situation of irrational application was analyzed.Results Antibacterial drugs included 350 cases of oral dosage form(mainly cefaclor,accounting for 23.66%)and 235 cases of intravenous administration(mainly cefuroxime sodium,accounting for 18.72%).There were many types,mainly respiratory system infections,accounting for 82.71%.There were 369 cases of single drug in the prescription containing antibiotics[236 cases were single oral(48.56%),133 cases were single intravenous(27.37%)],and the dual drugs were mainly sequential drugs,with intravenous cefuroxime sodium and oral cefuroxime esters dominated,accounting for 11.11%.Rational drug use in antibiotic prescriptions was 334 cases,accounting for 68.72%,and irrational drug use was 152 cases,accounting for 31.28%.Unreasonable drug use was mainly due to non-indicative drug use,followed by unreasonable usage and dosage and drug selection.Conclusion There are irrational application of antibacterial drugs in pediatrics,mainly including non-indicative drug use,unreasonable usage and dosage,and unreasonable drug selection.
