乳腺超声影像特征与乳腺恶性肿块的相关性分析

Correlation analysis between breast ultrasound imaging features and malignant breast masses

目的探究乳腺超声影像特征与乳腺恶性肿块的相关性。方法回顾性分析2019年5月至2020年5月本院收治的118例乳腺肿块患者的临床资料,按照乳腺肿块的良恶性分为良性肿块组(n=60)与恶性肿块组(n=58)。患者均行乳腺超声检查,采用单因素及多因素Logistic分析影响乳腺恶性肿块的高危因素。结果两组不同边界、内部回声、后方回声、阻力指数及周围回声占比比较差异无统计学意义。恶性肿块组年龄≥40岁、单发肿块、形态不规则、非平行于皮肤生长、肿块内部微钙化及肿块内穿支血流占比均高于良性肿块组,差异有统计学意义(P<0.05)。经多因素Logistic回归分析显示,年龄≥40岁、单发肿块、形态不规则及肿块内部微钙化为乳腺恶性肿块的高危因素(P<0.05)。结论年龄≥40岁、单发肿块、形态不规则及肿块内部出现微钙化为乳腺恶性肿块的高危因素,该类患者需引起足够重视,加强各项检查,以综合性分析及判断肿块良恶性。

Objective To explore the correlation between breast ultrasound imaging features and malignant breast masses.Methods The clinical data of 118 patients with breast lumps admitted to our hospital from May 2019 to May 2020 were retrospectively analyzed.They were divided into benign lump group(n=60)and malignant lump group(n=58)according to the benign and malignant breast lumps.All patients underwent breast ultrasonography,and univariate and multivariate Logistic analysis were used to analyze the high-risk factors of malignant breast masses.Results There was no significant difference in the proportion of different boundaries,internal echoes,rear echoes,resistance index and surrounding echoes between the two groups.The proportion of age≥40 years old,single mass,irregular shape,nonparallel to skin growth,microcalcification inside the mass and perforator blood flow in malignant lump group were higher than those in benign lump group,and the difference was statistically significant(P<0.05).Multivariate Logistic regression analysis showed that age≥40 years old,single mass,irregular shape and microcalcification inside the mass were high-risk factors of malignant breast masses(P<0.05).Conclusion Age≥40 years old,single mass,irregular shape and microcalcification inside the mass are high risk factors for malignant breast mass,sufficient attention should be paid to these patients,and various examinations should be strengthened to comprehensively analyze and judge the benign and malignant of the mass.
