基于临床管理路径表的全程营养干预对食管癌同步放化疗患者营养状态及生命质量的影响

Influence of whole-course nutritional intervention based on clinical management pathway table on nutritional status and quality of life in patients with esophageal cancer undergoing concurrent chemoradiotherapy

目的研究基于临床管理路径表的全程营养干预对食管癌同步放化疗期间患者营养状态及生命质量的影响。方法选取2018年3月至2020年6月于本院行同步放化疗的80例食管癌患者作为研究对象,按干预方案不同分为研究组和对照组,每组40例。对照组予以常规营养干预,观察组予以基于临床管理路径表的全程营养干预,比较两组干预前后主观整体营养状况评定量表(PG-SGA)分级情况、营养指标、生命质量。结果干预前,两组PG-SGA分级、血红蛋白(Hb)水平、清蛋白(ALB)水平、前清蛋白(PAB)水平、体重指数(BMI)、生命质量核心量表(QLQ-C30)评分比较差异无统计学意义;干预后,研究组PG-SGA分级优于对照组,Hb水平、ALB水平、PAB水平、BMI、QLQ-30评分均高于对照组,差异有统计学意义(P<0.05)。结论基于临床管理路径表的全程营养干预有助于减轻食管癌同步放化疗期间造成的负面影响,改善患者身体营养状态,提升其生命质量,值得临床推广应用。

Objective To study the effects of whole-course nutritional intervention based on clinical management pathway table on nutritional status and quality of life of patients with esophageal cancer undergoing concurrent chemoradiotherapy.Methods 80 patients with esophageal cancer who underwent concurrent chemoradiotherapy in our hospital from March 2018 to June 2020 were selected as the research subjects,and they were divided into study group and control group according to different intervention schemes,with 40 cases in each group.The control group was given routine nutrition intervention,while the observation group was given the whole-course nutritional intervention based on the clinical management pathway table,the patient-generated subjective global assessment(PG-SGA)classification,nutritional indicators and quality of life between the two groups before and after the intervention were compared.Results Before intervention,there was no significant difference in PG-SGA classification,hemoglobin(Hb)level,albumin(ALB)level,prealbumin(PAB)level,body mass index(BMI)and quality of life core scale(QLQ-C30)score between the two groups;after intervention,the PG-SGA classification of the study group was better than that of the control group,and the Hb level,ALB level,PAB level,BMI and QLQ-30 score were higher than those of the control group,the difference was statistically significant(P<0.05).Conclusion Whole-course nutritional intervention based on clinical management pathway table helps to reduce the negative impact during concurrent chemoradiotherapy for esophageal cancer,improve body nutritional status and quality of life,which is worthy of clinical promotion and application.
