盐酸右美托咪定复合依托咪酯在无痛人工流产中的麻醉效果观察

Anesthetic effect of dexmedetomidine hydrochloride combined with etomidate in painless induced abortion

目的探讨无痛人工流产中应用盐酸右美托咪定与依托咪酯复合麻醉的效果。方法抽取信阳市中医院2018年1月至2021年10月收治的1 100例无痛人工流产患者,按随机数字表法分为对照组与观察组,每组550例。对照组应用依托咪酯麻醉,观察组应用盐酸右美托咪定复合依托咪酯麻醉。比较两组麻醉效果、时间指标、不良反应发生率、术后疼痛评分,以及术前与术中生命体征指标。结果观察组麻醉优良率(98.55%,542/550)高于对照组(92.55%,509/550),P<0.05;与对照组比较,观察组手术时间、入睡时间、苏醒时间短,不良反应发生率低,术后疼痛评分低(P<0.05)。术前,两组心率、平均动脉压、脉搏血氧饱和度比较差异未见统计学意义(P>0.05);术中,观察组上述指标与术前比较差异未见统计学意义(P>0.05),但与对照组术中比较差异有统计学意义(P<0.05)。结论无痛人工流产术中应用盐酸右美托咪定与依托咪酯复合麻醉,可提高麻醉效果,维持术中生命体征稳定,缩短时间指标,减轻术后疼痛,减少不良反应。

Objective To investigate the effect of dexmedetomidine hydrochloride and etomidate combined anesthesia in painless induced abortion.Methods A total of 1100 patients who underwent painless abortion in Xinyang Hospital of Traditional Chinese Medicine from January 2018 to October 2021 were divided into control group and observation group by random number table method,with 550 patients in each group.The control group was anesthetized with etomidate,and the observation group was anesthetized with dexmedetomidine hydrochloride combined with etomidate.The anesthetic effect,time index,adverse reaction rate,postoperative pain score,and preoperative and intraoperative vital signs were compared between the two groups.Results The excellent and good rate of anesthesia in the observation group(98.55%,542/550)was higher than that in the control group(92.55%,509/550),P<0.05.Compared with the control group,the operation time,sleep time and recovery time of the observation group were shorter,the incidence of adverse reactions was lower,and the postoperative pain score was lower(P<0.05).Before operation,there was no significant difference in preoperative heart rate,mean arterial pressure and pulse blood oxygen saturation between the two groups(P>0.05);during operation,there was no significant difference between the preoperative and postoperative above indicators in the observation group(P>0.05),but there was a significant difference between the observation group and the control group(P<0.05).Conclusions The application of dexmedetomidine hydrochloride and etomidate combined anesthesia in painless induced abortion can improve the anesthetic effect,maintain the stability of vital signs during operation,shorten the time index,reduce postoperative pain,and reduce adverse reactions.
