益气养阴活血方联合常规治疗对扩张型心肌病患者的临床疗效

Clinical effects of Yiqi Yangyin Huoxue Formula combined with conventional treatment on patients with dilated cardiomyopathy

目的考察益气养阴活血方联合常规治疗对扩张型心肌病患者的临床疗效。方法80例患者随机对照组和观察组,每组40例,对照组给予常规治疗(参照2018年版《中国扩张型心肌病诊断和治疗指南》),观察组在对照组基础上加用益气养阴活血方,疗程12周。检测临床疗效、NYHA分级、6 MWT、MLHFQ评分、Lee氏心衰评分、心超指标(LVEF、LVEDD、LVESD,IVS、LVPWT)、BNP、TGF-β、安全性指标(血尿常规、肝肾功能、心电图)变化。结果观察组总有效率高于对照组(P<0.05)。治疗后,观察组心功能分级优于对照组(P<0.05),6 MWT更长(P<0.05),MLHFQ评分、Lee氏心衰评分、LVEDD、LVESD、BNP、TGF-β更低(P<0.05),LVEF更高(P<0.05),但2组IVS、LVPWT比较,差异无统计学意义(P>0.05)。2组未发现明显不良反应。结论益气养阴活血方联合常规治疗可安全有效地缓解扩张型心肌病患者临床症状,增强心功能,可能与改善心室重构有关。

AIM To investigate the clinical effects of Yiqi Yangyin Huoxue Formula combined with conventional treatment on patients with dilated cardiomyopathy.METHODS Eighty patients were randomly assigned into control group(40 cases)for 12-week intervention of conventional treatment complying with Chinese Guidelines for Diagnosis and Treatment of Dilated Cardiomayopathy(2018 Edition),and observation group(40 cases)for 12-week intervention of both Yiqi Yangyin Huoxue Formula and conventional treatment.The changes in clinical efficacy,NYHA grade,6 MWT,MLHFQ score,Lee’s heart failure score,cardiac ultrasound indices(LVEF,LVEDD,LVESD,IVS,LVPWT),BNP,TGF-βand safety indices(routine blood and urine,liver and kidney functions,electrocardiogram)were detected.RESULTS The observation group demonstrated higher total effective rate than the control group(P<0.05).After the treatment,the observation group displayed better NYHA grade than the control group(P<0.05),along with longer 6 MWT(P<0.05),lower MLHFQ score,Lee’s heart failure score,LVEDD,LVESD,BNP,TGF-β(P<0.05),and higher LVEF(P<0.05),but no significant differences in IVS and LVPWT were found between the two groups(P>0.05).No obvious adverse reactions were observable in the two groups.CONCLUSION For the patients with dilated cardiomyopathy,Yiqi Yangyin Huoxue Formula combined with conventional treatment can safely and effectively alleviate clinical symptoms and enhance cardiac functions,which may contribute to the improvement of ventricular remodeling.
