香芹酚联合TPF方案治疗口腔癌临床观察

Clinical Observation of Carvacrol Combined with TPF Regimen in the Treatment of Oral Cancer

目的探讨香芹酚联合多西他赛+顺铂+氟尿嘧啶(TPF方案)治疗口腔癌的临床疗效。方法选取华北理工大学附属医院2017年12月至2020年12月收治的口腔癌患者102例,按随机抽签法分为观察组和对照组,各51例。两组患者均给予TPF方案治疗,观察组患者加用香芹酚注射液静脉滴注。两组均以3周为1个周期,连续治疗3个周期。结果观察组患者客观缓解率和疾病控制率分别为74.51%和88.24%,明显高于对照组的54.90%和70.59%(P<0.05)。治疗后,观察组患者的癌胚抗原、鳞状细胞癌相关抗原、糖链抗原50水平均明显低于对照组;核因子E_(2)相关因子2(Nrf2)、核苷酸结合寡聚化结构域样受体3(NALP3)、核因子κB(NF-κB)水平均明显低于对照组;T淋巴细胞亚群CD_(3)^(+),CD_(4)^(+)水平及CD_(4)^(+)/CD_(8)^(+)均明显高于对照组,CD_(8)^(+)水平明显低于对照组(P<0.05)。观察组与对照组不良反应发生率相当(17.65%比11.76%,P>0.05)。结论香芹酚联合TPF方案治疗口腔癌,能降低肿瘤标志物及Nrf2、NALP3、NF-κB水平,改善免疫功能。

Objective To investigate the clinical efficacy of carvacrol combined with docetaxel+cisplatin+fluorouracil(TPF)in the treatment of oral cancer.Methods A total of 102 patients with oral cancer admitted to the Affiliated Hospital of North China University of Science and Technology from December 2017 to December 2020 were selected and divided into the observation group and the control group by the random lottery method,with 51 cases in each group.The patients in the two groups were treated with the TPF regimen,on this basis,the patients in the observation group were treated with carvacrol injection intravenously.Both groups were treated for three consecutive cycles with three weeks as a cycle.Results The objective remission rate(OOR)and disease control rate(DCR)in the observation group were 74.51% and 88.24%,which were significantly higher than 54.90% and 70.59% in the control group(P<0.05).After treatment,the levels of carcinoembryonic antigen(CEA),squamous cell carcinoma-associated antigen(SCC)and sugar chain antigen 50(CA50)in the observation group were significantly lower than those in the control group,the levels of nuclear factor E_(2) related factor 2(Nrf2),nucleotide-binding oligomerization domain-like receptor 3(NALP3),nuclear factor-κB(NF-κB)in the observation group were significantly lower than those in the control group,the levels of T lymphocyte subsets including CD_(3)^(+),CD_(4)^(+)and CD_(4)^(+)/CD_(8)^(+)in the observation group were significantly higher than those in the control group,while the level of CD_(8)^(+)in the observation group was significantly lower than that in the control group(P<0.05).The incidence of adverse drug reactions in the observation group was similar to that in the control group(17.65%vs.11.76%,P>0.05).Conclusion Carvacrol combined with the TPF regimen in the treatment of oral cancer can reduce tumor markers and Nrf2,NALP3 and NF-κB levels,and improve immune function.
