经皮椎间孔镜下髓核摘除术对腰椎间盘突出症患者术后康复及JOA评分的影响

Influence of Percutaneous Transforaminal Endoscopic Discectomy on Postoperative Rehabilitation and JOA Score of Patients with Lumbar Disc Herniation

目的探讨经皮椎间孔镜下髓核摘除术(PTED)对腰椎间盘突出症(LDH)患者术后康复及日本骨科学会(JOA)评分的影响。方法选取我院2020年1月至2021年3月期间收治的80例LDH患者,根据手术方式不同分为传统组与PTED组各40例。传统组应用椎体开窗髓核摘除手术治疗,PTED组行PTED。对比两组的手术指标、术后康复指标及腰部功能JOA评分。结果PTED组的手术时间、术后下床活动时间与术后住院时间均短于传统组,术中出血量低于传统组,切口长度短于传统组,术后4 h疼痛评分低于传统组(P<0.05)。术后3个月、6个月,PTED组的JOA评分均高于传统组(P<0.05)。结论PTED治疗LDH的手术时间短、切口小、创伤小、术后康复快,可显著改善患者腰部功能。

Objective To explore the influence of percutaneous transforaminal endoscopic discectomy(PTED)on postoperative rehabilitation and Japanese Orthopaedic Society(JOA)score of patients with lumbar disc herniation(LDH).Methods 80 LDH patients admitted to our hospital from January 2020 to March 2021 were divided into traditional group(n=40)and PTED group(n=40)according to different surgical methods.The traditional group received vertebral fenewing nucleus pulpotomy,and the PTED group received PTED.The operation indicators,postoperative rehabilitation indicators and JOA score of lumbar function were compared between the two groups.Results Compared with the traditional group,the operation time,postoperative out-of-bed activity time and hospitalization time of the PTED group were shorter,the intraoperative blood loss was lower,the incision length was shorter,and the pain score at 4 h after surgery was lower(P<0.05).3 months and 6 months after surgery,the JOA scores of the PTED group were higher than those of the traditional group(P<0.05).Conclusions PTED in the treatment of LDH has the advantages of short operation time,small incision,small trauma and rapid postoperative recovery,which can significantly improve the lumbar function of patients.
