哮喘-慢阻肺重叠综合征的中医体质特点研究

Study on TCM Constitution Characteristics of Asthma-COPD Overlap Syndrome

目的探讨哮喘-慢阻肺重叠综合征(asthma-COPD overlap syndrome,ACOS)患者的中医体质,并研究不同中医体质的特点,为治疗哮喘-慢阻肺重叠综合征提供数据参考。方法收集本院诊断为ACOS的患者298例,就诊的时间为2010年12月-2019年6月。所有患者均进行问卷调查,问卷调查内容包括患者人口学资料、中医体质量表和临床症状。患者根据中华中医药学会《中医体质分类和判定》对中医体质进行分类。记录患者的吸烟指数、肺功能指标FEV_(1)/FVC和年急性哮喘发作的次数。结果ACOS人群的占主要比例的体质类型分别是阳虚质(119例)、气虚质(80例)、淤血质(44例)和阴虚质(30例)。阳虚质和气虚质类型患者的吸烟指数要高于淤血质和阴虚质患者,阳虚质的FEV_(1)/FVC(%)水平最低,淤血质FEV_(1)/FVC(%)水平最高,阳虚质和气虚质患者的年急性发作次数要高于淤血质和阴虚质患者。结论阳虚质、气虚质、淤血质和阴虚质为哮喘-慢阻肺重叠综合征的主要中医体质,中医体质类型与吸烟指数、肺功能和年急性发作次数密切相关。

Objective To investigate the TCM physique of asthma-COPD overlap syndrome(ACOS)patients,and studied the characteristics of different TCM physicies,providing data reference for treating asthma-COPD overlap syndrome.Methods 298 patients diagnosed with ACOS were collected from December 2010 to June 2019 in our hospital.All patients underwent questionnaire survey including patient demographic data,TCM quality and clinical symptoms.Patients were classified in TCM in accordance with the Chinese Medicine Society"Traditional Chinese Medicine Classification and Judgment".Record the number of times of smoking index,lung function index FEV_(1)/FVC and an acute asthma.Results The physical type of the ACOS population was the type of yang immersion(119 cases),a gas extraction(80 cases),and the brown(44 cases)and the yin fumet(30 cases).The smoking index of the yang humeral and qi deficiency type patients were higher than those of the squeextive and yin extract,the yang fumes FEV_(1)/FVC(%)was the lowest level,the squeextive FEV_(1)/FVC(%)was the highest level,the number of emergency episodes in the yang humex and qi deficiency patients were higher than those of the squeezance and yin extract.Conclusion Yang fumes,qi deficiency,squeezance and yin extract are the main TCM physique of ACOS,TCM physique type is closely related to smoking index,lung function and emergency episodes.
