无痛人流术中不同部位静脉注射丙泊酚对注射痛的影响研究

Influence of Intravenous Injection of Propofol at Different Sites During Painless Artificial Abortion on Injection Pain

目的:探讨无痛人流术中不同部位静脉注射丙泊酚对注射痛的影响。方法:选取2017年7-9月于湖南省株洲市中心医院接受无痛人流术终止妊娠的132例患者作为研究对象,以抽签的方式分为三组。手背静脉组47例,肘前静脉组44例,颈外静脉组41例。比较三组注射痛程度和局部安全性。结果:颈外静脉组注射疼痛程度明显轻于手背静脉组和肘前静脉组,差异有统计学意义(P<0.05)。颈外静脉组穿刺点出血、皮下瘀血发生率均低于手背静脉组和肘前静脉组,一次性穿刺成功率高于手背静脉组和肘前静脉组,差异有统计学意义(P<0.05)。结论:在无痛人流术中,颈外静脉注射丙泊酚的效果更好,一次性静脉穿刺成功率高,可减轻患者注射疼痛程度,降低穿刺点出血、皮下瘀血发生率,局部安全性更高。

Objective:To investigate the influence of intravenous injection of propofol at different sites on injection pain during painless artificial abortion.Methods:A total of 132 patients who underwent painless artificial abortion to terminate pregnancy in Zhuzhou Central Hospital of Hunan Province from July to September in 2017 were selected as the study subjects,and they were divided into three groups by the method of drawing lots.There were 47 cases in the dorsal hand vein group,44 cases in the antecubital vein group,and 41 cases in the external jugular vein group.The injection pain degree and local safety in three groups were compared.Results:The injection pain degree in the external jugular vein group was significantly lower than that in the dorsal hand vein group and the antecubital vein group,and the difference was statistically significant(P<0.05).The incidences of puncture point bleeding and subcutaneous congestion in the external jugular vein group were lower than those in the dorsal hand vein group and the antecubital vein group,and the one-time puncture success rate was higher than that in the dorsal hand vein group and the antecubital vein group,and the difference was statistically significant(P<0.05).Conclusion:In painless artificial abortion,external jugular vein injection of propofol has a better effect and a high success rate of one-time venipuncture,it can reduce the injection pain degree of patients,reduce the incidences of puncture point bleeding and subcutaneous congestion,and has a higher local safety.
