项目管理模式在老年衰弱患者出院计划关键任务中的实施效果

Effect of project management model in the implementation of discharge plan of senile debilitation patients

目的探讨项目管理模式在老年衰弱患者出院计划关键任务中的实施效果,为出院计划关键任务的实施提供依据。方法选择江苏大学附属人民医院老年医学科60例老年衰弱患者,分为观察组和对照组各30例。对照组实施常规出院护理,观察组实施项目管理模式,根据临床评估情况在出院护理中加入出院准备计划关键任务,开展风险评估,制定风险应对策略并实施计划。结果观察组患者出院需求干预率高于对照组(P<0.05);观察组患者医院-社区-家庭过渡期各类护理安全不良事件低于对照组(P<0.05);观察组家庭照顾者居家照护和康复锻炼知识掌握率高于对照组(P<0.05)。结论项目管理模式确定出院计划目标人群的筛选和需求评估,通过多团队合作、制定个性化出院计划等一系列干预措施,能有效降低医院-社区-家庭过渡期各类护理安全不良事件的发生,大幅提高家庭照顾者基本照护技能掌握率,提高患者出院信心,保证患者获得高质量出院服务体验。

Objective To explore the effect of project management model in the implementation of discharge plan of senile debilitation patients and provide the basis of discharge plan. Methods 60 senile debilitation patients form the Department of Geriatric, Affiliated Hospital of Jiangsu University were selected and randomly divided into the observation group and control group, with 30 patients in each group. The control group was given routine discharge care, while the observation group was given the discharge preparation plan based on clinical assessment. Results The intervention rate of discharge demand in the observation group was higher than that in the control group(P<0.05), the number of nursing safety adverse events during the hospital-community-family transition period was lower than that in the control group(P<0.05). The mastery rate of home care and rehabilitation exercise knowledge of family caregivers in the observation group was higher than that in the control group(P<0.05). Conclusions Project management model to determine the screening and demand assessment of the target population of the discharge plan, through multi-team cooperation, personalized discharge plan and a series of interventions, effectively reduce the hospital-community-family transition all kinds of nursing safety adverse events, greatly improve the family caregivers basic care skills, improve patients discharge confidence, ensure patients get high quality discharge service experience.
