心脏术后患者运动恐惧与康复运动依从性现状及其相关性

Effect of kinesiophobia on exercise compliance with out-of-hospital cardiac rehabilitation in patients after cardiac surgery

目的 了解心脏外科术后患者运动恐惧、心脏康复运动依从性的现状,分析两者之间的关系。方法 采用一般资料调查表、心脏病患者运动恐惧量表(tampa scale for kinesiophobia heart,TSK-SV Heart)、心脏康复运动依从性问卷对本科室收治的心脏外科术后212例患者进行调查。采用Pearson相关性分析探讨心脏外科术后患者运动恐惧与康复运动依从性的关系。结果 212例患者TSK-SV Heart得分为(45.91±9.41)分;心脏康复运动依从性问卷得分为(21.99±4.65)分,69例(32.55%)依从率高,143例(67.45%)依从率低。患者运动恐惧总分以及各维度得分均与运动依从性呈负相关关系(均P<0.01)。结论 心脏外科术后患者运动恐惧水平较高,心脏康复运动依从性不理想。患者运动恐惧程度越高,心脏康复运动依从性越低。医护人员应加强对运动恐惧的关注与评估,及时提供干预与指导,以提高患者心脏康复运动依从性,增强术后康复效果。

Objective To explore the current situations of kinesiophobia and compliance with cardiac rehabilitation exercises in patients after cardiac surgery and analyse the relationship between them. Methods The investigation was conducted among212 patientsafter cardiac surgery using general information questionnaire, Tampa scale for kinesiophobia heart(TSK-SV Heart) and cardiac rehabilitation exercise compliance questionnaire. Pearson correlation analysis and multiple linear stepwise regression analysis were used to screen the influencing factors of rehabilitation exercise compliance in the patients. Results The score of TSK-SV Heart was(45.91±9.41) in all of 212 patients after cardiac surgery, at a high level. The compliance score of cardiac rehabilitation exercise was(21.99±4.65).Ofthe 212 patients, 69(32.55%) had a higher compliance rate and 143(67.45%) had a lower rate, with anoverall compliance rate at a medium level. The total score of TSK-SV Heart and the score of each dimension were negatively correlated with exercise compliance(all P<0.01). Conclusions The level of kinesiophobia in patients after cardiac surgery is high, in a manner of the higher the level of kinesiophobiathe lower the compliance of cardiac rehabilitation exercise. The compliance of cardiac rehabilitation exercise requires to be further improved. It is suggested that medical staff should strengthen the attention and evaluation of kinesiophobia.Interventionalmeasures and guidance should be provided in time, so as to improve the compliance of patients with cardiac rehabilitation and enhance the effect of postoperative cardiac rehabilitation.
