摘要
目的 研究联丝蛋白(synemin,SYNM)、乙醛脱氢酶1(aldehyde dehydrogenase1,ALDH1)、半乳糖凝集素–3(Galectin–3)在大肠癌中的表达及与病理特征的相关性。方法 选取2017年8月至2019年8月在台州市第一人民医院病理科保存的大肠癌组织标本74例,设为大肠癌组;另选取同期在台州市第一人民医院病理科保存的正常大肠黏膜(距离癌组织>5cm)74例,设为对照组。将其进行免疫组织化学染色,评估SYNM、ALDH1、Galectin–3阳性表达情况。随访至2020年8月,统计两组患者的存活率。结果 与对照组相比,大肠癌组SYNM阳性表达下降,ALDH1、Galectin–3阳性表达升高,差异均有统计学意义(P<0.05)。与肿瘤最大径<2.0cm患者相比,2.0cm~5.0cm、>5.0cm患者的ALDH1、Galectin–3阳性表达较高;高分化程度患者SYNM阳性表达高于低、中分化程度患者;与高分化程度患者相比,中、低分化程度患者ALDH1、Galectin–3阳性表达较高;中分化程度患者ALDH1、Galectin–3阳性表达高于低分化程度患者,差异均有统计学意义(P<0.05)。TNM分期Ⅲ、Ⅳ期患者SYNM阳性表达低于Ⅰ、Ⅱ期患者,ALDH1、Galectin–3阳性表达高于Ⅰ、Ⅱ期患者;与无淋巴结转移患者相比,有淋巴结转移患者SYNM阳性表达较低,ALDH1、Galectin–3阳性表达较高,差异均有统计学意义(P<0.05)。SYNM阴性表达和ALDH1、Galectin–3阳性表达患者生存率分别低于SYNM阳性表达和ALDH1、Galectin–3阴性表达患者,差异均有统计学意义(P<0.05)。结论 SYNM、ALDH1、Galectin–3在大肠癌组织中表达异常,其阳性表达高低与患者肿瘤最大径、分化程度、TNM分期、淋巴结转移有关,三者联合检测在大肠癌患者预后中具有重要意义。
Objective To study the expression of synemin(SYNM), aldehyde dehydrogenase1(ALDH1) and Galectin–3 in colorectal cancer and their correlation with pathological features. Methods 74 cases of colorectal cancer tissue specimens preserved in the Department of Pathology, Taizhou First People’s Hospital from August 2017 to August 2019 were selected and seted as colorectal cancer group, and 74 cases of normal colorectal mucosa(distance from cancer tissue >5.0cm) preserved in Department of Pathology, Taizhou First People’s Hospital at the same time were selected and seted as control group. Immunohistochemical staining was used to evaluate the positive expression of SYNM, ALDH1 and Galectin–3. The patients were followed up until 2020 to August, and the survival rate was counted. Results Compared with the control group, the positive expression of SYNM decreased and the positive expression of ALDH1 and Galectin–3 increased in the colorectal cancer group, all differences were statistically significant(P<0.05). The positive expressions of ALDH1 and Galectin–3 in patients with tumor diameter ≤2.0cm and >5.0cm were higher than those in patients with tumor diameter ≤2.0cm;the positive expression of SYNM in patients with high differentiation was higher than that in patients with low and medium differentiation;the positive expressions of ALDH1 and Galectin–3 in patients with medium and low differentiation were higher than those in patients with high differentiation;the positive expressions of ALDH1 and Galectin–3 in patients with moderate differentiation were significantly higher than those in patients with low differentiation, all differences were statistically significant(P<0.05). The positive expression of SYNM in TNM stage Ⅲ and Ⅳ patients was lower than that in stage Ⅰand Ⅱ patients, and the positive expression of ALDH1 and Galectin–3 was higher than that in stage Ⅰand Ⅱpatients;Compared with patients without lymph node metastasis, patients with lymph node metastasis had lower positive expression of SYNM and higher positive expression of ALDH1 and Galectin–3, all differences were statistically significant(P<0.05). The survival rates of patients with negative expression of SYNM and positive expression of ALDH1 and Galectin–3 were lower than those with positive expression of SYNM and negative expression of ALDH1 and galectin–3, respectively, all differences were statistically significant(P<0.05). Conclusion The expression of SYNM, ALDH1 and Galectin–3 was abnormal in colorectal cancer. The positive expression of SYNM, ALDH1 and Galectin–3 was related to tumor diameter, differentiation degree, TNM stage and lymph node metastasis.
