摘要
目的研究调控白细胞介素(IL)-6对视神经脊髓炎谱系病(NMOSD)患者辅助性17型T细胞(Th17)/调节性T细胞(Treg)比值失衡的影响。方法收集2019年1月至2020年12月就诊的9例AQP4-IgG阳性的NMOSD患者为NMOSD组和同期8例健康志愿者为对照组,收集并培养外周血单个核细胞(PBMC)后,分别予以IL-6或IL-6受体抑制剂共培养,采用流式细胞术检测Th17和Treg比例,流式微球检测技术检测调控IL-6水平后对Th17、Treg、IL-17、IL-10水平的影响。结果NMOSD组PBMC细胞添加IL-6共培养后,可见Th17比例及Th17/Treg比值增高(均P<0.05)。添加IL-6阻滞剂共培养后,IL-10水平下降,Treg表达增加,Th17/Treg比值下降(均P<0.05)。结论NMOSD患者PBMC中的外源性IL-6可以上调Th17比例而导致Th17/Treg比值失衡;但外源性IL-6受体阻滞剂可以升高IL-10水平,通过增加Treg比例恢复Th17/Treg比值平衡。
Aim To study the effect of regulating IL-6 on the imbalance of Th17/Treg cells in patients with neuromyelitis optica spectrum disorders.Methods A total of 9 patients with neuromyelitis optica spectrum disorder(NMOSD)who were hospitalized in the Uygur Autonomous Region People’s Hospital from January 2019 to December 2020 were collected.During the same period,8 healthy volunteers who underwent physical examination were selected as healthy controls.The peripheral blood mononuclear cells(PBMC)were collected and cultured,and then were co-cultured with IL-6 or IL-6 receptor inhibitor.The proportion of Th17 and Treg cells was detected by flow cytometry,and the levels of IL-17 and IL-10 in the supernatant were detected by cytometric bead array.The effect of regulating IL-6 level on Th17,Treg,IL-17,IL-10 were observed.Results The ratio of Th17 and Th17/Treg cells increased after co-culture with IL-6 in PBMC cells in the NMOSD group.After co-culture with IL-6 blocker,the level of IL-10 decreased,the expression of Treg cells increased,and the ratio of Th17/Treg cells decreased.Conclusion Exogenous IL-6 can up-regulate Th17 ratio in PBMCs of NMOSD patients by regulating IL-6 levels,leading to the imbalance of Th17/Treg cells,but exogenous IL-6 receptor blockers can increase IL-10 levels by increasing the proportion of Treg cells,thereby restoring the balance of Th17/Treg cells.
