经会阴超声结合经阴道超声在诊断肛周不同来源性感染中的价值

The Value of Perineal Ultrasound Combined with Transvaginal Ultrasound in Diagnosing Infections from Different Origins

目的:探讨经会阴超声结合经阴道超声在诊断肛周不同来源性感染中的价值。方法:择取我院2020年2月至2021年7月疑似肛周疾病患者70例,男性患者仅给予经会阴超声诊断,女性患者给予经会阴超声+经阴道超声诊断,评估其诊断价值。结果:1)60例肛腺源感染患者中,术前超声诊断脓肿符合率为100.00%、主瘘管为94.92%、支管为83.33%、内口为86.67%、外口为100.00%。2)10例非肛腺源感染患者中,术前超声诊断和术后病理完全符合,包含会阴尿道瘘2例、肛周表皮样囊肿3例、藏毛囊肿伴感染3例、肛周化脓性汗腺炎1例、血栓性外痔1例。结论:针对肛周不同来源性感染,经会阴超声、经阴道超声联合诊断可对炎症予以分型,评估其炎症病变程度,还具有无痛和成本低等优势,诊断价值较高,值得推广应用。

Objective To explore the value of perineal ultrasound(PNU)combined with transvaginal ultrasound(TVU)in diagnosing infections from different origins. Methods Enrolled 70patients with suspected perianal diseases treated in author’s hospital(2020-02-2021-07)into this study:male patients received PNU inspection,female,PNU+TVU;then,evaluated both diagnostic value. Results 1)In the 60cases of infection from anal-glandular orgins,preoperatively ultrasound diagnosing as abscess,the accordance rate was 100.00%,including main fistula vessel’s 94.92%,branch vessel’s 83.33%,inner orifice’s 86.67%exit-orifice’s 100%.2)In the 10cases of infection from non-anal-glandular origins,their preoperatively ultrasound diagnosed one,including 2cases of perinum-urethra fistula,3cases of perianal epidemoid,3cases of piliferous cyst,1case of perianal purulent hydradenitis,and 1case of thrombotic external hemorrhoids. Conclusion PNU combined with TVU can type perianal inflammation from different origins,and estimate inflammatory degree of lesion;It has such advantages as painless,low cost,and high diagnostic value,thus is worth to be popularized to apply it.
