股内收肌群激痛点小针刀治疗膝骨关节炎的临床效果

Clinical effect of small akupotomye on the trigger points of the adductor femoral muscle group in the treatment of knee osteoarthritis

目的观察通过股内收肌群激痛点小针刀治疗膝骨关节炎的临床效果。方法选择2020年10月至2021年10月在西安交通大学附属红会医院康复医院门诊就诊的膝骨关节炎患者80例为研究对象,按照随机数字表法将其分为对照组和观察组,各40例。对照组给予体外冲击波治疗,观察组在对照组基础上给予股内收肌群激痛点小针刀治疗,4周为1个疗程。采用中医证候疗效、西安大略和麦克马斯特大学骨关节炎指数(WOMAC)量表及Lysholm膝关节评分量表评价膝关节功能。结果治疗后,观察组中医证候疗效优于对照组,差异有统计学意义(P<0.05)。治疗后两组WOAMAC评分均降低,且观察组低于对照组,差异有统计学意义(P<0.05)。治疗后两组Lysholm评分均升高,且观察组高于对照组,差异有统计学意义(P<0.05)。结论股内收肌群小针刀配合体外冲击波同时处理膝关节内外侧肌肉,可改善膝关节的动力性不平衡,有效缓解膝骨关节炎疼痛,改善功能障碍,提高生活质量,且优于单纯使用冲击波进行股外侧肌肉治疗,值得在临床中推广应用。

Objective To observe the clinical effect of small akupotomy therapy on knee osteoarthritis through stimulating trigger points of adductor femoral muscle group.Methods A total of 80 patients with knee osteoarthritis treated in the outpatient department of the Rehabilitation Hospital of the Honghui Hospital Affiliated to Xi’an Jiaotong University from October 2020 to October 2021 were selected as the research objects.According to the random number table method,they were divided into control group and observation group,with 40 cases in each group.The control group was treated with extracorporeal shock wave,and the observation group was treated with small akupotomy on the trigger points of adductor femoral muscle group on the basis of control group.Four weeks was as a course of treatment.The syndrome efficacy of traditional Chinese medicine,Wester Ontario and McMaster Universities osteoarthritis(WOMAC)index and the Lysholm knee rating scores were used to evaluated knee function.Results After treatment,the curative effect of traditional Chinese medicine syndrome in the observation group was better than the control group,and the difference was statistically significant(P<0.05).After treatment,the WOAMAC scores of the two groups were decreased,and the observation group was lower than the control group,and the differences were statistically significant(P<0.05).After treatment,Lysholm scores of the two groups were increased,and the observation group was higher than the control group,and the difference was statistically significant(P<0.05).Conclusion The acupotomy of adductor femoral combined with extracorporeal shock wave can simultaneously deal with the internal and external muscles of the knee joint,improve the dynamic imbalance of the knee joint,effectively alleviate the pain of knee osteoarthritis,improve dysfunction,and improve the quality of life.It is better than the treatment of lateral femoral muscle with shock wave alone,and it is worth popularizing in clinical practice.
