首发抑郁症住院患者低体质量率及其影响因素

目的 探讨在首发抑郁症住院患者低体质量的发生率及临床相关性.方法 选取2020年1月至2021年10月在本院心身医学科住院的211例首发抑郁症患者,根据体质量指数(BMI)分为低体质量组(37例)和非低体质量组(174例).采用临床结构式访谈,汉密尔顿抑郁量表(HAMD)、汉密尔顿焦虑量表(HAMA)进行诊断和评估.结果 211例抑郁症患者低体质量患病率为17.5%.低体质量组患者自杀意念、HAMD评分高于非低体质量组,差异有统计学意义(P<0.05);TG/HDL比值、低密度脂蛋白水平低于非低体质量组,差异有统计学意义(P<0.05).多因素Logistic分析显示HAMD评分、TG/HDL值、低密度脂蛋白水平为低体质量的危险因素(P<0.01).多元线性回归分析显示HAMD评分、收缩压、尿酸水平、高密度脂蛋白水平、低密度脂蛋白水平对BMI的预测作用有统计学意义(F=19.877,P<0.01,R^(2)=0.311).结论 HAMD评分、收缩压、尿酸水平、高密度脂蛋白水平、低密度脂蛋白水平对BMI有一定预测作用.

Objective To investigate the incidence and clinical correlation of underweight in hospitalized patients with first-episode depression.Methods A total of 211 inpatients with first-episode depression were divided into underweight group(37 cases)and non-underweight group(174 cases)according to body massindex(BMI).Diagnosis and evaluation were performed by Psychiatrists using Clinical structured interview,Hamilton Depression Scale(HAMD)and Hamilton Anxiety Seale(HAMA).Results±The prevalence of underweight in 211 patients with depression was 17.5%.Compared with non-underveight group,the incidences of suicidal ideation and HAMD scores were higher in the underweight group(P<0.05),while TG/HDL values and lowdensity lipoprotein levels were lower(P<0.05).Multi-factor logistics analysis showed that HAMD score,TG/HDL value and lowdensity lipoprotein level were significantly correlated with underweight(P<0.01),.Muliple linear regresion analysis showed that HAMD scores,systolice blood pressure,uric acid levels,high density lipoprotein levek and low-density lipoprotein levels had statistical significance in predicting BMI(F=19.877,P<0.01,R=0.311).Conclusion HAMD scores,ystolic blood pressure levek,uric acid leveks,high density lipoprotein levels and lowdensity lipoprotein levels have certain predictive fects on BMI.
