摘要
目的探究微创硬通道钻孔引流手术治疗高血压性脑出血的效果。方法98例高血压性脑出血患者,随机分为对照组与实验组,每组49例。对照组给予开颅血肿清除术治疗,实验组给予微创硬通道钻孔引流手术治疗。比较两组血肿清除效果,治疗前后美国国立卫生研究院卒中量表(NIHSS)评分、日常生活能力评定量表(ADL)评分,治疗效果,并发症发生情况与手术时间。结果实验组患者的血肿清除效果显著优于对照组,差异具有统计学意义(P<0.05)。治疗前,两组患者的NIHSS、ADL评分比较,差异无统计学意义(P>0.05);治疗后,实验组患者的NIHSS评分(20.2±2.4)分显著低于对照组的(27.6±3.7)分,ADL评分(47.6±5.2)分显著高于对照组的(29.3±3.9)分,差异具有统计学意义(P<0.05)。实验组治疗总有效率为95.9%,显著高于对照组的77.6%,差异具有统计学意义(P<0.05)。实验组患者的并发症发生率4.1%低于对照组的16.3%,手术时间(104.9±6.3)min显著短于对照组的(176.8±9.7)min,差异具有统计学意义(P<0.05)。结论高血压性脑出血患者采用微创硬通道钻孔引流手术治疗,能够明显缩短手术时间,并发症明显减少,能够更好的清除血肿,促使神经功能的恢复,患者生活质量得到显著提升,应用价值较高。
Objective To investigate the effect of minimally invasive hard channel drilling and drainage in the treatment of hypertensive cerebral hemorrhage.Methods A total of 98 patients with hypertensive cerebral hemorrhage were randomly divided into control group and experimental group,with 49 cases in each group.The control group was treated with craniotomy and hematoma removal,and the experimental group was treated with minimally invasive hard channel drilling and drainage.The hematoma removal effect,National Institutes of Health stroke scale(NIHSS)score,activites of daily living(ADL)score before and after treatment,therapeutic effect,complications and operation time were compared between the two groups.Results The hematoma removal effect of the experimental group was significantly better than that of the control group,and the difference was statistically significant(P<0.05).Before treatment,there was no statistically significant difference in NIHSS and ADL scores between the two groups(P>0.05).After treatment,the NIHSS score(20.2±2.4)points of the experimental group was significantly lower than(27.6±3.7)points of the control group,the ADL score(47.6±5.2)points was significantly higher than(29.3±3.9)points of the control group.All the differences were statistically significant(P<0.05).The total effective rate of treatment in the experimental group was 95.9%,which was significantly higher than 77.6%in the control group,and the difference was statistically significant(P<0.05).The complication rate 4.1%in the experimental group was lower than 16.3%in the control group,and the operation time(104.9±6.3)min was significantly shorter than(176.8±9.7)min in the control group.All the differences were statistically significant(P<0.05).Conclusion Minimally invasive hard channel drilling and drainage for patients with hypertensive cerebral hemorrhage can obviously shorten the operation time,reduce complications,better remove hematoma,promote the recovery of neurological function,and improve the quality of life of patients,and have a high application value.
