摘要
目的探究布地奈德雾化治疗时机对支气管肺发育不良(BPD)早产儿的影响。方法85例支气管肺发育不良早产儿,依据布地奈德雾化时机不同分为甲组(出生<28 d,43例)、乙组(出生≥28 d,42例)。对比两组患儿的治疗及住院时间、临床治疗效果、并发症发生率。结果甲组患儿的机械通气时间、氧疗时间、住院时间分别为(14.24±2.31)、(21.58±4.07)、(34.32±6.04)d,均短于乙组的(22.98±3.25)、(24.55±4.16)、(45.28±6.72)d,差异具有统计学意义(P<0.05)。甲组患儿总有效率为93.02%,高于乙组的76.19%,差异具有统计学意义(P<0.05)。两组患儿并发症发生率对比,差异无统计学意义(P>0.05)。结论对支气管肺发育不良早产儿予以早期布地奈德雾化治疗的效果优于晚期布地奈德雾化治疗,能够缩短机械通气时间、氧疗时间及住院时间,并提升临床治疗效果,而且不增加并发症发生率,安全可靠。
Objective To investigate the effect of the timing of nebulization of budesonide on preterm infants with bronchopulmonary dysplasia(BPD).Methods A total of 85 premature infants with bronchopulmonary dysplasia were divided into group A(within 28 d after birth,43 cases)and group B(≥28 d after birth,42 cases)according to the timing of nebulization of budesonide.The treatment and hospitalization time,clinical treatment effect and complication rate were compared between the two groups.Results The mechanical ventilation time,oxygen therapy time and hospitalization time in group A were(14.24±2.31),(21.58±4.07)and(34.32±6.04)d,which were shorter than(22.98±3.25),(24.55±4.16),and(45.28±6.72)d in group B,and the differences were statistically significant(P<0.05).The total effective rate in group A was 93.02%,which was higher than 76.19%in group B,and the difference was statistically significant(P<0.05).There was no statistically significant difference in complication rate between the two groups(P>0.05).Conclusion The effect of early budesonide nebulization therapy on preterm infants with bronchopulmonary dysplasia is better than that of late budesonide nebulization therapy,which can shorten the time of mechanical ventilation,oxygen therapy and hospitalization time,and improve the clinical treatment effect without increasing the incidence of complications,and is safe and reliable.
