基于体检大数据的低剂量苯暴露职业人群空腹血糖水平调查研究

摘要 目的探讨长期低剂量苯暴露对职业接触工人血糖的影响及与血常规指标的关系。方法收集试验组参与职业健康体检的8195名苯暴露者空腹血糖及血常规指标检测数据和8195名未接触苯健康体检者作为对照组空腹血糖检测数据,应用统计学方法分析进行规律探讨。结果试验组空腹血糖(fasting blood glucose,FBG)受损者总体检出率为5.08%(416人),其中男性为5.72%(348人),女性为3.22%(68人)。试验组男性血糖(5.17±0.96 mmol/L)高于女性血糖(5.03±0.62 mmol/L),差异有统计学意义(P<0.05);试验组20~50岁年龄段男性血糖高于女性(P=0.00);17~19岁、51~65岁年龄段血糖差异无统计学意义(P>0.05)。试验组FBG与年龄(r=0.24,P<0.01)、工龄(r=0.16,P<0.01)、血白细胞计数(r=0.06,P<0.01)呈正相关,FBG与血血小板计数及苯接触水平无明显相关(P>0.05)。试验组血糖测定结果异常率随着工龄增长而增长。试验组空腹血糖(中位数为5.00 mmol/L)水平大于对照组(中位数为4.60 mmol/L),差异有统计学意义(P<0.01)。结论长期低剂量苯接触工人血糖总体水平呈上升趋势。长期低剂量苯接触生产工作人群血糖异常率未发现高于中国成人总体情况,工龄增长过程血糖呈上升趋势可能是与年龄增长有关,而与低剂量苯暴露叠加的关系暂时未有明确相关性。 Objective To explore the effect of benzene exposure on the blood glucose of occupationally exposed workers and the relationship with blood routine indicators.Methods 8195 benzene exposed persons who participated in the occupational health examination were tested for fasting blood glucose and blood routine indexes,and the law was discussed using statistical methods.Results The overall detection rate of impaired fasting blood glucose(FBG)was 5.08%(416 persons),of which 5.72%(348 persons)were males and 3.22%(68 persons)were females.The blood glucose of the male group(5.17±0.96mmol/L)was higher than that of the female group(5.03±0.62mmol/L),and the difference was statistically significant(P<0.05).;Males aged 20~50 are higher than females(P=0.00);17~19,51~There was no statistically significant difference in blood glucose between 65 years of age(P>0.05).FBG correlated with age(r=0.24,P<0.01),length of service(r=0.16,P<0.01),and white blood cells count(r=0.06,P<0.01),but not platelet count or benzene exposure(P>0.05).The abnormal rate of blood glucose test results of exposed workers increased with the increase of working age.Conclusion The abnormal rate of blood glucose among workers exposed to long-term low-dose benzene exposure is not higher than that of adults in China.The rising trend of blood glucose during working age may be related to the increase of age and the superimposed relationship between low-dose benzene exposure and there is no clear correlation for the time being.
