特瑞普利单抗致孤立性促肾上腺皮质激素缺乏症一例

A case of isolated adrenal corticotropin deficiency caused by Toripalimab

目的报道1例由特瑞普利单抗导致的免疫相关不良反应。方法对1例57岁男性肺鳞癌患者给予特瑞普利单抗治疗后出现垂体相关不良反应进行分析及评价。结果患者给予糖皮质激素治疗后症状改善,半年后不良反应复发。结论对于由于免疫治疗引起的肾上腺功能不全需长期糖皮质激素替代治疗,免疫治疗过程中监测激素水平及定期随访必不可缺。

Objective To report a case of immune-related adverse reactions caused by Toripalimab.Methods A 57-year-old male patient with lung squamous cell carcinoma was analyzed and evaluated for pituitary-related adverse reactions after treatment with Toripalimab.Results The patient's clinical symptoms improved after glucocorticoid treatment,and the adverse reaction recurred after half a year.Conclusion Long-term glucocorticoid replacement therapy is needed for adrenal insufficiency caused by immunotherapy.Monitoring hormone levels and regular follow-up during immunotherapy are indispensable.
