Invisalign矫治器辅助治疗牙性错颌畸形的效果及对龈沟液炎性因子水平的影响

Effect of Invisalign Appliance in the Adjuvant Treatment of Dental Malocclusion and the Level of Inflammatory Factors in Gingival Crevicular Fluid

目的研究Invisalign矫治器辅助治疗牙性错颌畸形的临床价值。方法回顾性分析安阳市第六人民医院2017年3月至2019年3月收治的100例牙性错颌畸形患者的临床资料,按治疗方案不同分组,其中将以Damon Q自锁托槽治疗的记为对照组(N=47),以Damon Q自锁托槽+Invisalign矫治器治疗的记为研究组(N=53)。比较两组治疗前、治疗6个月后夹角(SN-MP、Y轴角)水平、牙周指数[牙龈指数(GI)、菌斑指数(PLI)、探诊深度(PD)]、龈沟液炎性因子[肿瘤坏死因子-α(TNF-α)、白介素-1β(IL-1β)]水平、复诊间隔时间、生活质量(OHRQOL评分)。结果治疗6个月后两组SN-MP、Y轴角增大,研究组大于对照组,差异有统计学意义(P<0.05)。治疗6个月后两组GI、PLI、PD及龈沟液TNF-α、IL-1β水平比较,差异无统计学意义(P>0.05)。研究组复诊间隔时间长于对照组,差异有统计学意义(P<0.05)。治疗6个月后研究组OHRQOL评分高于对照组,差异有统计学意义(P<0.05)。结论Invisalign矫治器辅助治疗牙性错颌畸形效果较为理想,可促进夹角改善,与单纯Damon Q自锁托槽比较,不增加对牙周指数、牙周炎症的影响,延长复诊间隔时间,提升患者生活质量。

Objective To study the clinical value of Invisalign appliance in theadjuvant treatment of dental malocclusion Objective The clinical data of 100 patients with dental malocclusion in our hospital from March 2017 to March 2019 were retrospectively analyzed.According to different treatment plans,47 patients who were treated with Damon Q self-locking brackets were classified as the control group,and 53 cases treated with Damon Q self-locking brackets and Invisalign appliances were classified as the study group.The included angle(SN-MP,Y-axis angle)level and periodontal index[gingival index(GI),plaque index(PLI),probing depth(PD)],gingival crevicular fluid inflammatory factors[tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β)]levels before and 6 months after treatment,interval of return visit,quality of life(OHRQOL score)were compared between the two groups.Results After 6 months of treatment,the SN-MP and Y-axis angles of the two groups increased,and the study group was larger than the control group(P<0.05).There were no significant differences in the levels of GI,PLI,PD and gingival crevicular fluid TNF-α,IL-1βbetween the two groups after 6 months of treatment(P>0.05).The interval between visits in the study group was longer than that in the control group(P<0.05).After 6 months of treatment,the OHRQOL score of the study group was higher than that of the control group(P<0.05).Conclusion The Damon Q self-locking bracket combined with Invisalign appliance is effective in patients with dental malocclusion,it can promote the improvement of included angle.Compared with Damon Q self-locking bracket alone,it can prolong the interval between visits,and improve the quality of life of patients.and not increase the influence on periodontal index and periodontal inflammation.
