血液净化治疗对有机磷中毒患者CHE、CK-MB及IL6水平的影响

Effects of blood purification treatment on CHE,CK-MB and IL6 levels in patients with organophosphorus poisoning

目的分析血液净化治疗对有机磷中毒患者胆碱酯酶(CHE)、肌酸激酶同工酶(CK-MB)及白介素-6(IL-6)水平的影响。方法选取2015年1月至2022年1月于太和县人民医院治疗的102例有机磷中毒患者的临床资料,根据不同治疗方式将其分为观察组(n=53,血液净化治疗)、对照组(n=49,常规治疗)。比较两组临床疗效、炎性因子[IL-6、降钙素原(PCT)、C反应蛋白(CRP)]、心肌酶谱[CK-MB、肌酸激酶(CK)、肌钙蛋白(cTnI)]、CHE水平以及并发症。结果观察组临床总有效率(94.34%)高于对照组临床总有效率(80.43%),差异有统计学意义(P<0.05)。治疗后,两组IL-6、PCT、CRP水平均出现下降,其中观察组下降更为明显,差异均有统计学意义(P<0.05)。治疗后,两组CK、CK-MB、cTnI均出现下降,CHE水平上升,其中观察组变化更为明显,差异均有统计学意义(P<0.05)。观察组并发症总发生率(11.32%)低于对照组总发生率(28.57%),差异有统计学意义(P<0.05)。结论有机磷中毒患者采取血液净化治疗疗效确切,可有效降低患者的IL-6、PCT、CRP水平,改善患者的CK、CK-MB、cTnI及CHE水平,且具有一定的安全性,值得临床应用推广。

Objective To analyze the effects of blood purification treatment on Cholinesterase(CHE),creatine kinase isoenzymes(CK-MB)and interleukin-6(Il-6)levels in patients with organophosphorus poisoning.Methods The clinical data of 102 patients with organophosphorus poisoning treated in Taihe County People’s Hospital from January 2015 to January 2022 were selected,they were divided into the observation group(n=53,blood purification treatment)and the control group(n=49,conventional treatment).The clinical efficacy,levels of inflammatory factors,CHE level and complications were compared between the two groups.Results The total clinical effective rate(94.34%)of the observation group was higher than that of the control group(80.43%),and the difference was statistically significant(P<0.05).After treatment,the levels of IL-6,PCT,and CRP in the two groups decreased,and the observation group decreased more significantly,with statistical significance(P<0.05).After treatment,the levels of CK,CK-MB and cTnI in the two groups decreased,and the level of CHE increased,among which the changes in the observation group were more obvious,and the differences were statistically significant(P<0.05).The total incidence of complications in the observation group(11.32%)was lower than that in the control group(28.57%),and the difference was statistically significant(P<0.05).Conclusion Blood purification treatment for patients with organophosphorus poisoning is effective,can effectively reduce the levels of IL-6,PCT,CRP,and improve the levels of CK,CK-MB,cTnI and CHE in patients,and has certain safety,which is worthy of clinical application and promotion.
