基于数据挖掘分析马晓昌教授治疗冠心病PCI术后病人的用药规律

Professor Ma Xiaochang′s Medication Regularity in Treating Coronary Heart Disease after PCI Based on Data Mining

目的总结并分析马晓昌教授治疗冠心病经皮冠状动脉介入(PCI)术后病人的用药规律。方法收集并整理马晓昌教授临床治疗PCI术后的医案,录入中医传承计算平台V3.0,对治疗PCI术后处方中所含中药进行频次、四气、五味、归经分析,以药物关联规则分析及聚类分析。结果共纳入医案352份,涉及中药169味,中药四气以寒、温为主,五味以甘、辛、苦为主,归经以肝、肺为主。核心药物组合为延胡索-郁金、丹参-延胡索等。核心处方由丹参、鸡血藤、陈皮、茯苓、郁金、延胡索、红景天、薏苡仁、车前草等药物组成。结论马晓昌教授认为PCI术后病人的病机为“本虚标实”,本虚多为气虚、阳虚和阴虚,以气虚为主,标实指痰、瘀、热三者相互胶结,以“活血化瘀、祛痰清热、益气温阳滋阴”为治疗大法。

Objective To summarize and analyze the medication rules of Professor Ma Xiaochang in the treatment of coronary heart disease after percutaneous coronary intervention(PCI).Methods The clinical medical records of Ma Xiaochang in treating patients after PCI were collected and sorted out,and the prescriptions of the medical records were recorded into the Traditional Chinese Medicine Inheritance Computer platform V3.0.The frequency statistics,four characters,five tastes,meridian tropism analysis,association rules analysis,and core prescription mining were carried out.Results A total of 352 medical records were recorded,involving 169 kinds of traditional Chinese medicine.The four natures mainly include cold and warm,the five flavors mainly include sweet,spicy,and bitter.Liver and lung were the main meridian tropism.The core drug combination includes Rhizome Corydalis-Curcumae Radix,Salvia miltiorrhiza-Rhizome Corydalis,etc.The core prescription was composed of Salvia miltiorrhiza,Caulis Spatholobi,Pericarpium Citri Reticulatae,Poria cocos,Curcuma aromatica,Corydalis yanhusuo,Rhodiola,Coix seed,Asiatic plantain and other high-frequency drugs.Conclusion Professor Ma Xiaochang believes that the pathogenesis of patients after PCI is"fundamental deficiency and severe external symptoms".Fundamental deficiency mostly refers to Qi deficiency,Yang deficiency,and Yin deficiency,mainly Qi deficiency.Severe external symptoms refers to the cementation of phlegm,blood stasis and heat.The main treatment method is"activating blood circulation to remove blood stasis,eliminating phlegm and clearing heat,supplementing Qi,warming Yang,and nourishing Yin".
