前庭性偏头痛发作期及间歇期患者血清5-羟色胺、降钙素基因相关肽水平相关性研究

Correlation Study of Blood Serotonin and Calcitonin Gene-related Peptide Levels in Patients with Vestibular Migraine during Attack and Intermittent Period

目的探究前庭性偏头痛(VM)发作期与间歇期患者血清5-羟色胺(5-hydroxytryptamine,5-HT)和降钙素基因相关肽(calcitonin gene-related peptide,CGRP)水平的相关性。方法选取2019年1月至2020年6月我院收治30例VM患者作为VM组,并按照患者疾病分期将其分为两个亚组(发作组与间歇组),另选取同时期我院健康体检者30例作为对照组,检测3组研究对象血清5-HT和CGRP,并行相关性分析。结果发作组患者5-HT和CGRP水平高于间歇组和对照组患者(P<0.05);患者发作期与间歇期与5-HT和CGRP均存在相关性,5-HT和CGRP回归系数分别为0.855和0.874,(P<0.05);将患者发作期与间歇期作为阳性结局绘制ROC曲线,结果表明:患者5-HT、CGRP均对阳性结局具有较强敏感性(P<0.05)。5-HT对阳性结局的最佳截断值为149.72 ng/mL,≥149.72 ng/mL时敏感度为100.00%,特异度为100.00%;CGRP对阳性结局的最佳截断值为67.68 ng/L,>67.68 ng/L时敏感度为100.00%,特异度为100.00%。结论VM患者发作与血清5-HT和CGRP水平存在明显相关性,可将其作为评估患者病情的重要依据,为患者早期治疗方案制订提供参考。

Objective To explore the correlation between the level of serotonin 5-HT and calcitonin gene related peptide in patients with vestibular migraine attacks and intermittent periods.Method Select 30 patients with vestibular migraine in our hospital from January 2019 to June 2020 as the vestibular migraine group,and divide the patients into two subgroups according to the disease stage,and select the health checkup of our hospital during the same period 30 patients were used as the control group.The blood 5-HT and CGRP of the three groups were tested,and the correlation analysis of Logistics was performed.Result The levels of 5-HT and CGRP in the seizure group were higher than those in the intermittent group and the control group(P<0.05).There was a correlation between the onset and intermittent periods of patients with 5-HT and CGRP,and the regression coefficients of 5-HT and CGRP were 0.855 and 0.874,respectively(P<0.05).ROC curves drew the seizure period and interval period as positive outcome,and the results showed that both 5-HT and CGRP had strong sensitivity to positive outcome(P<0.05).The best cut-off value of 5-HT for positive outcome was 149.72 ng/mL,≥149.72 ng/mL 100.00%sensitivity and specificity 100.00%;and the best cut-ff of CGRP was 67.68 ng/L,≥67.08 ng/L sensitivity 100.00%and specificity 100.00%.Conclusion There is a significant correlation between the onset of vestibular migraine and the levels of blood 5-HT and CGRP,which can be used as an important basis for evaluating the patient's condition and providing a reference for the formulation of early treatment plans for patients.
